小梁切除联合超声乳化术治疗原发性闭角型青光眼合并白内障效果分析

摘　　要：目的:观察小梁切除术联合白内障超声乳化术及人工晶体植入术治疗原发性闭角型青光眼合并白内障患者的临床疗效及安全性。方法:对96例(96眼)闭角型青光眼合并白内障患者进行回顾性调查,随机分为观察组和对照组,每组48例(48眼)。观察组行小梁切除术联合白内障超声乳化术及人工晶体植入术,对照组行单纯白内障超声乳化及人工晶体植入术。随访12个月,观察两组术前及术后眼压、视力情况、中央前房深度、周围前房深度、术后降压药物种数、并发症等。结果:两组患者术后眼压、视力、中央前房深度、周围前房深度均比术前明显改善(P<0.05),观察组增加程度高于对照组(P<0.05)。观察组术后降压药使用种数、并发症数量均少于对照组。结论:小梁切除术联合白内障超声乳化术及人工晶体植入术治疗闭角型青光眼合并白内障能降低眼压、提高视力,减少术后并发症的发生,是有效而安全的方法。Objective： To investigate the effect and safety of trabeculectomy combined with phacoemulsification and in traocular lens for primary angle-closes glaucoma with cataract. Methods： Retrospective study was performed in 96 cases（96 eyes） which were divided into observation group and control group by half. Patients in the observation group were treated with trabeculectomy combined with phacoemulsification and intraocular lens implanation, the control group were treated with simple phacoemulsification and intraocular lens implanation. Intraocular pressure （IOP）, visual acuity, thick of lens, anterior cham- ber depth around, IOP-lowering drugs and complications were observed in all patients followed up in 12 months. Results.. IOP, visual acuity, thick of lens, anterior chamber depth around were increased postoperatively compared with those preoperative（P 〈0.05）, the increase in the observation group was more significant. Compared with control group, the observation group showed less kind of IOP-lowering drugs and few complications. Conclusion： Trabeculectomy combined with phacoemulsification and intraocular lens implanation is effective and safe treatment for primary angel-closure glaucoma with cataract patients, which can reduce intrao- cular pressure, enhance vision and reduce the postoperative complications.
