婴幼儿反复喘息与血清维生素 D水平相关性研究

摘　　要：目的：探讨婴幼儿反复喘息与血清25-羟维生素D水平之间的相关性，并探讨婴幼儿反复喘息的相关危险因素。方法收集2012年10月至2013年3月在我院哮喘门诊就诊的反复喘息婴幼儿（6～36个月）61例为研究对象；另随机选取同期保健门诊健康婴幼儿（6～36个月）97例为对照组。采用化学发光免疫法测定各组儿童血清25羟-维生素 D的水平，完成病史采集、查体和实验室检查。结果反复喘息组患儿血清25-羟维生素D平均水平（14.38±4.35）μg/L，低于对照组血清25-羟维生素D平均水平（24.07±11.05）μg/L（F＝33.39，P＜0.001）；反复喘息组血清25-羟维生素D≤20μg/L的发生率90％，而对照组11％，前者明显高于后者（ P＜0.001）；多因素回归分析显示特应性过敏体质、吸烟接触史和血清25-羟维生素D≤20μg/L是婴幼儿反复喘息的危险因素（ OR＝9.89，95％CI 3.15～23.11）。结论婴幼儿反复喘息与血清低水平的25-羟维生素D相关。Objective To investigate the level of serum 25（OH）D in children with recurrne t wheezing under the ageo f 36 months,and to explore ht e relta ed risk factors for recurrent wheezing under36 monthsof age.Methods Sixty-one children aged from 6～36 months who visited the asthma clinic from November 2012 to March 2013 with recurrent wheezing were selected for the recurrent wheezing group.Meanwhile ,97 children aged from 6～36 months who visited the healthcare clinic were selected for the control group.The serum 25 （ OH） D concentrations were measured by chemiluminescence immunoassay.Mde iac l history,physical examina-tion andl ba oratory data were collected.Results Teh serum 25（OH）D level in the recurrent wheezing group [（14.38 ±4.35）μg/L]were lower than the control group[（24.07 ±11.05）μg/L],Showing a signfi icant differenec （F=33.39,P〈0.001）.The incidence rate of plasm a 25（OH）D≤20μg/L in the recurrent wheezing group（90%）w ere higher than that of the control group（11%）,showing a significant difference（P〈0.001）. Serum 25（OH） D ≤20 μg/L,allergy physique and smoking contact history were the risk factors for children with recurrent wheezing under 36 months（OR=9.89,95%CI 3.15～23.11）.Conclusion Lowre serum vita-min D level might be assocait ed with the children with recurrent wheezing under 36 months.
