化疗间歇期患者PICC健康管理知识和方式需求调查

摘　　要：目的了解化疗间歇期患者对PICC健康管理知识、方式的需求。方法 2013年6—12月,采用自行设计的化疗患者PICC健康管理需求调查表,按照分层随机抽样法,对新疆乌鲁木齐市某三级甲等医院即将携带PICC导管出院的213例患者进行问卷调查,调查的主要内容有:(1)化疗间歇期PICC患者健康管理知识的需求内容,包括PICC的基本知识、PICC导管维护知识、PICC带管日常活动知识、导管并发症处理的指导及导管维护地点等;(2)患者在化疗间歇期对健康管理方式的需求。结果化疗间歇期患者PICC健康管理知识需求得分排前3位的是:导管维护地点(4.40±0.81)分、并发症处理的指导(4.39±0.74)分和出院后带管的注意事项(4.39±0.74)分;患者希望医院采取定期电话随访(78.9%)、QQ视频指导(43.2%)和短信提醒(36.6%)的健康管理服务;46.9%的患者希望医院网络医疗健康管理平台网页定期发布有关PICC健康管理知识的信息以供阅览;患者QQ视频指导、电话随访和短信提醒的频次选择最高的为1次/周、随访期限选择最多的为3个月,随访的时间段主要为晚上(20:00—21:00)。结论化疗间歇期PICC带管患者对健康管理知识需求较强烈,希望享有个性化、具体化的健康管理方式。医院在对化疗间歇期PICC患者制定健康管理计划时应增加相应的措施,以满足患者健康管理需求。Objective To survey the needs of PICC health management knowledge and approach of patients during the intermission of chemotherapy. Methods With stratified random sampling, we had used a self-designed questionnaire about health management knowledge and approach to survey 213 cases of patients discharged with PICC from June to December in 2013. Results Catheter maintenance locations, guidance on how to treat complications and precautions for discharging with PICC topped the health management knowledge list which scored 4.40±0.81, 4.39±0.74, 4.39±0.74 respectively and regular telephone follow-up （78.9%）, QQ video guide （43.2%） and SMS reminders （36.6%） the list of health management services. Totally 46.9% of the patients hoped to acquire PICC health management knowledge from healthcare platform of the hospital regularly and patients wanted to enjoy health management services through QQ video guide, telephone follow-up and SMS reminders once a week, at most a period of three months and majorly at night. Conclusion Patients with PICC during chemotherapy intermission have strong demands of health management knowledge and they need health management services such as personalized health management, rationalized follow-up periods and specific follow-up frequency. Specific health management programs for patients with PICC in chemotherapy intermission could meet their healthcare needs.
