甲氨蝶呤联合米非司酮保守治疗异位妊娠的疗效分析

摘　　要：目的 探析临床上采用甲氨蝶呤联合米非司酮保守治疗异位妊娠的临床疗效.方法 将2010年10月至2012年11月期间收治的120例异位妊娠患者为研究对象,将其随机分为对照组和治疗组,每组60例,对照组患者仅给予米非司酮治疗,治疗组在对照组治疗基础上肌注甲氨蝶呤,两组患者均给予维生素B6减轻胃肠道反应,给予抗生素预防感染,对比分析两组疗效和不良反应发生情况,同时观察及对比两组患者平均住院时间、血β-HCG值、包块吸收情况.结果 治疗组治疗总有效率明显优于对照组（P＜0.05）,住院时间明显缩短（P＜ 0.05）;包块吸收情况明显优于对照组（P＜0.05）;血β-HCG值明显小于对照组（P＜0.05）,两组患者治疗后不良反应发生率比较差异无统计学意义（P＞0.05）.结论临床上采用甲氨蝶呤联合米非司酮保守治疗方案治疗异位妊娠疗效显著,不良反应少,明显缩短住院时间,安全性高,此种治疗方案值得在临床上广泛的推广和应用.Objective To analyze the clinical efficacy of methotrexate and mifepristone combination treatment for ectopic pregnancy.Methods From October 2010 to November 2012,120 cases of ectopic pregnancy treated in our hospital selected in this study were randomly divided into two groups namely the control and treatment groups,60 cases in each group,the control group was given only mifepristone,the treatment group was treated on the basis of intramuscular methotrexate,two groups were both given vitamin B6 to reduce gastrointestinal side effects and antibiotics to prevent infection,efficacy and adverse events in two groups were comparatively analyzed,and we compared patients of two groups with an average duration of hospitalization,blood β-HCG values and mass absorption.Results The total effective rate in treatment group was better than that of the control group （P ＜ 0.05）,length of hospital stay was significantly shorter （P ＜ 0.05）; mass absorption was also better than that in the control group （P ＜ 0.05）; serum β-HCG values were significantly lower than those of the control group （P ＜ 0.05）,in these two groups,the incidence of adverse reactions after the treatment showed no statistical significant difference （P ＞ 0.05）.Conclusion The clinical use of methotrexate combined with mifepristone for ectopic pregnancy treatment has significant curative effects,with less adverse reaction,significantly shorter hospital stay and high safety,which is worthy of wide clinical promotion and application.
