激光灼闭术与剥脱术治疗大隐静脉曲张的效果比较分析

摘　　要：目的探讨激光灼闭术与传统剥脱术治疗大隐静脉曲张的疗效。方法选择2010年3月至2011年3月保定市第二医院收治的80例(80条患肢)大隐静脉曲张患者作为研究对象,根据治疗方法不同分为传统剥脱术组和激光灼闭术组,每组40例。比较两组患者的手术时间、术中出血量、术后并发症、术后住院时间以及术后6、12个月的复查结果。结果激光灼闭术组患者手术时间、术中出血量、术后住院时间显著少于传统剥脱术组(t=2.012、14.685、22.749,P<0.05),术后并发症比较差异无统计学意义(χ2=0.41,P>0.05),术后6个月和12个月时,两组患者能够长期站立,血管肿胀情况、酸胀情况、静脉曲张团、湿疹及溃疡减轻或者消失,激光灼闭术组患者的复发率显著低于传统剥脱术组(χ2=4.8,P<0.05)。结论激光灼闭术是一种安全有效的治疗大隐静脉曲张的手术方法,其能显著缩短患者的手术时间、住院时间、减少术后出血量,复发率低,值得临床推广应用。Objecive To explore the effect of endovenous laser therapy and traditional stripping on treatment of varicose veins. Methods Total of 80 cases （80 affected limbs） with varix saphena magna from Baoding City Second Hospital during Mar. 2010 and Mar. 2011 were collected as the research objects, accord- ing to treatment methods,they were divided into conventional stripping operation group and endovenous laser therapy group ,40 patients in each group. The operation time, intraoperative bleeding volume, postoperative complications,postoperative hospital stay and postoperative 6,12 months follow-up results of the two groups were compared. Results The operation time intraoperative bleeding volume, postoperative hospitalization time of the endovenous laser therapy were significantly less than the traditional decortication treatment group （t = 2. 012.14. 685,22. 749,P 〈 0.05 ）, there was no statistically significant difference in operation time and postoperative complications between the two groups （ X2 = 0.41, P 〉 0.05 ）. After 6 and 12 months, the patients in both grottps were able to stan~ for a long time, blood vessel swelling, acid bilges, varices group, eczema and ulcer reduced or disappeared, The recurrent rate of endovenotls laser therapy group was lower than that of the traditional stripping group（ X2 = 4.8 ,P 〈 0. 05 ）. Conclusion Endovenous laser therapy is a safe and effective operation method for treatment of varix saphena magna,whieh can significantly shorten the hospitalization time, reduce the amount of bleeding after operation, with low recurrence rate, thus is worthy of clinical application.
