磁共振弥散加权成像联合LAVA动态增强扫描诊断肝脏局灶性病变的临床价值

摘　　要：目的探讨磁共振弥散加权成像(DWI)联合快速容积采集(LAVA)序列动态增强扫描诊断肝脏局灶性病变的临床价值。方法回顾性分析2012年2月至2013年9月简阳市人民医院放射科收治的56例肝脏局灶性病变患者的临床资料,分析DWI和LAVA动态增强扫描的特点以及联合检测的临床价值。结果原发性肝细胞癌、肝脏转移瘤以及肝血管瘤的表观弥散系数平均值差异有统计学意义(P<0.05);3种病变的最大上升斜率、峰值、最大下降斜率及正性增强积分之间的比较,差异均有统计学意义(P<0.05);3种病变曲线类型比较差异有统计学意义(P<0.05)。DWI联合LAVA动态增强扫描的符合率均高于单项检查,且与单项检查符合率比较差异有统计学意义(P<0.05)。结论不同局灶性病变的DWI和LAVA动态增强扫描表现不同,DWI联合LAVA动态增强扫描诊断对提高诊断准确率具有重要意义。Objective To investigate the clinical value of diffusion-weighted imaging （DWI） and dynamic contrast-enhanced magnetic resonance imaging with liver acquisition with volume acceleration （LAVA） sequence in the diagnosis of focal hepatic lesions. Methods The clinical data of 56 cases patients with focal hepatic lesions were analyzed retrospectively. The character of DWI and LAVA were analyzed, and the clinical value of combined detection was summarized. Results The apparent diffusion coefficient average value of primary hepatocellular carcinoma,liver metastasis carcinoma and hepatic hemangioma were inconsistent, and the difference was statistically significant （P 〈 0.05 ）. Primary hepatocellular carcinoma, liver metastasis carcinoma and hepatic hemangioma had significant difference in maximum slope of increase, peak value,maximum slope of decrease and positive enhancement integral （P 〈 0.05 ）. The three types of curve distributed with significant difference（P 〈 0.05 ）. The coincidence rate of DWI combined with LAVA were higher than the individual check, the difference was statistically significant （ P 〈 0.05 ）. Conclusion Different kinds of focal hepatic lesions show different information in DWI and LAVA, DWI combined with LAVA has important significance to improve the accuracy in the diagnosis of focal hepatic lesions.
