CAT评分对评估慢性阻塞性肺疾病病情及急性加重诊断的意义

摘　　要：目的探讨CAT评分对于反映COPD患者病情及评估急性加重的意义。方法选取88例COPD-AE患者,分别于住院时及出院前进行CAT评分,并于住院后行肺功能检测,比较稳定期及急性加重期CAT分值差异,比较低CAT评分组及高评分组FEV1%pred及急性加重频率的区别,观察稳定期CAT值与FEV1%pred及此前1年内急性加重次数的相关性。结果急性加重期COPD患者CAT评分显著高于稳定期(P<0.05)。高CAT评分组FEV1%pred明显低于低CAT评分组,1年内急性加重次数多于低CAT评分组(P<0.05)。稳定期CAT评分与FEV1%pred无明显相关性(P>0.05),但与此前1年内急性加重次数呈正相关(r=0.72,P<0.05)。结论结合CAT评分可更好反映COPD疾病全貌,CAT评分可增加COPD-AE诊断的客观性。Objective To discuss the evaluation value of chronic obstructive pulmonary disease assessment test（CAT）on diagnosis of chronic obstructive pulmonary disease （COPD）and judgment of COPD acute exacerbation （COPD-AE）. Methods CAT was carried out on 88 cases with COPD-AE when admitted and before discharged,pulmonary function test was also carried out for all cases.The difference of CAT score in acute exacerbation and stable stage was compared. Also FEV1% pred and frequency of acute exacerbation was compared between different CAT scoring groups.Meanwhile the relationship between CAT score in stable stage and FEYI% pred and times of acute exacerbation in last year were observed. Results CAT score in acute exacerbation stage was significantly higher than that in stable stage （P〈0.05）. FEV1% pred in high CAT group was significantly lower than in lower CAT group,the times of acute exacerbation in last year in high CAT group was higher than in lower CAT group （P〈0.05）.There was no relationship between CAT score in stable stage and FEV1% pred （P〉0.05）,but an obvious positive relationship between CAT score and the acute exacerbation times in last year exists （r=0.72,P〈0.05）. Conclusion As an independent evaluation factor, CAT scoring is helpful for diagnosis of COPD and judgment of COPD-AE.
