摘 要:目的探讨早产儿早期静脉营养在其生理性体重下降恢复时间、体重增长、所需住院时间、达所需追赶生长总热量时间中的作用及意义。方法选取2012年7月~2013年10月我科收治的采用静脉营养治疗的73例早产儿作为治疗组,2010年2月~2011年2月收治的未采用静脉营养治疗的73例早产儿作为对照组,治疗组出生24h内给予氨基酸.超过24h予以脂肪乳;观察两组早产儿生理性体重下降恢复时间、体重增长情况、所需住院时间、达所需追赶生长总热量时间。结果治疗组早产儿体重平均每日增加(22.27±7.44)g,对照组每日增加(19.56±7.17)g,两组差异有统计学意义(P〈0.05);两组住院时间及生理性体重下降恢复时间差异有统计学意义(P〈0.05);治疗组追赶生长所需总热量时间为(5.95±2.28)d,对照组为(7.25±2.03)d,两组差异有统计学意义(P〈0.01)。两组并发症发生率差异无统计学意义(P〉0.05)。结论静脉营养能促进早产儿生长发育,缩短住院时间,减少生理性体重下降,且临床应用较为安全.值得进一步推广。Objective To explore the effect and significance of early parenteral nutrition in the time of physiological weight loss recovery,weight gain,hospital stay,and the time to reach total calorics needed for catch-up growth. Methods 73 premature infants who were given parenteral nutrition admitted in our hospital from July 2012 to October 2013 were assigned to the treatment group,73 premature infants who were not given parenteral nutrition admitted from February 2010 to February 2011 were assigned to the control group.The treatment group was given amino acid within 24 hours after birth and fatty acid if exceeding 24 hours after birth.The time of physiological weight loss recovery,weight gain, hospital stay,and the time to reach total calories needed for catch-up growth were observed to analyze whether there was statistical difference between the two groups. Results The treatment group had weight gain of (22.27±7.44) g per day and the control group had weight gain of (19.56±7.17) g per day,with statistical difference (P〈0.05).The two groups had statistical difference in hospital stay and the time of physiological weight loss recovery (P〈0.05).The time to reach total calories needed for catch-up growth was (5.95±2.28) d in the treatment group and (7.25±2.03) d in the control group,with statistical difference (P〈0.01).The incidence of complications in two groups had no significant difference (P〉 0.05). Conclusion Parenteral nutrition can promote growth and development of premature infants,shorten hospital stay, reduce physiological weight loss and demonstrate safe clinical application,which is recommended for further clinical promotion.
