鼻内镜下双极电凝与传统填塞治疗鼻出血的效果比较

摘　　要：目的 比较鼻内镜下双极电凝与传统填塞治疗鼻出血的效果。方法选取本院2010年12月～2013年12月住院的鼻出血患者77例，按照数字随机法分为观察组和对照组，观察组40例患者采用鼻内镜下双极电凝治疗，对照组37例采用传统填塞治疗，比较两组的平均止血时间、平均住院时间及术后并发症发生情况，所有患者均随访3个月，对疗效进行评价。结果 观察组治愈率为97．50％，平均住院时间为（3．42±0．64）d；对照组治愈率为83．78％，平均住院时间为（6．55±1．31）d；观察组治疗效果明显好于对照组，平均住院时间明显短于对照组，差异有统计学意义（P〈0．05）。观察组治疗后综合征发生率为20．00％，黏膜变化发生率为15．00％；对照组治疗后综合征发生率为54．05％，黏膜变化发生率为56．76％；对照组并发症发生率明显高于观察组，差异有统计学意义（P〈0．05）。结论 鼻内镜下双极电凝止血具有操作简单、直视下操作的特点，能尽快找准出血点，止血效果好，而且可缩短治疗时间．是鼻出血治疗的有效方法。Objective To investigate the clinical efficacy of bipolar coagulation and traditional stuffing under endoscopic in treatment of epistaxis. Methods 77 cases of patients with epistaxis in our hospital December 2010 to December 2013 were divided into the observation group （40 cases） and control group（37 cases）,the observation group were treated by intranasal endoscopic bipolar coagulation therapy,the control group were treated by traditional stuffing.The average bleeding time,the average length of hospital stay and postoperative complications of two groups were analyzed. Results The cure rate of the observation group was 97.50%,higher than 83.78% of the control group （P〈0.05）;The mean length of stay of the observation group was （3.42±0.64） d,lower than （6.55±1.31） d of the control group （P〈0.05）.The incidence of syndrome and mucosal changes of observation group was 20.00% and 15.00%,lower than 54.05% and 56.76% of the control group （P〈0.05）. Conclusion Endoscopic bipolar coagulation is simple operation,characteristics of operation under direct vision,to identify the bleeding as soon as possible,a good hemostatie effect,can shorten the duration of treatment and is an effective method of treatment with nasal bleeding.
