四黄水蜜膏敷脐联合西药治疗肝硬化自发性腹膜炎随机平行对照研究

摘　　要：[目的]观察四黄水蜜膏敷脐联合西药治疗肝硬化自发性腹膜炎疗效。[方法]使用随机平行对照方法,将70例住院患者按病志号抽签简单随机分为两组。对照组35例常规西医综合,利尿、护肝解毒、维持水电解质及酸碱平衡及抗生素。治疗组35例四黄水蜜膏（黄连、黄柏、黄芩、大黄、冰片、乳香、血竭、没药各10g,苦参20g,伸筋草15g,透骨草20g）,研磨成粉,加适量凉水与蜂蜜,拌糊,外敷脐部及周围,再红外线灯照射脐部,微微加热,3∽4h/次,2次/d;西药治疗同对照组。连续治疗10d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈15例,显效16例,有效2例,无效2例,总有效率91.43%。对照组痊愈10例,显效12例,有效3例,无效10例,总有效率71.43%。治疗组疗效优于对照组（P〈0.05）。[结论]四黄水蜜膏敷脐联合西药治疗肝硬化自发性腹膜炎效果显著,值得推广。[ Objective ] To observe the four including yellow honey cream apply umbilical curative effect on liver cirrhosis spontaneous peritonitis. [ Methods ] Using random parallel control method, 70 cases of hospitalized patients into two groups according to random number table method. The control group of 35 cases of conventional western medicine comprehensive treatment. Respectively to the diuresis, protect liver detoxification, maintain water electrolyte and acid-base balance, and antibiotics. Four including yellow honey cream treatment group 35 examples （Huanglian, Huangbai, Huangqi, Dahuang, Bingpian, Ruxiang, Xuexie, Meiyao each 10g, Kushen 20g, Shenjineao 15g, Tougueao 20g） ;Grinding into powder, add right amount of cold water and honey, mix paste, topical navel and surrounding, infrared lamp irradiation time again, a little heat, 3 - 4 h/time, 2 times/d. Western medicine treatment with the control group. Continuous treatment of 10d to 1 course of treatment. Observation of clinical symptoms and adverse reactions. As a course treatment, judge curative effect. [ Results ] the treatment group （ 15 cases cured, 16 cases were markedly effective, effective in 2 cases, 2 had no effect, the total effective rate was 91.43%. The control group, 10 cases were cured, 12 eases were markedly improved, 3 eases, 10 cases were invalid, and the total effective rate 71.43%. The treatment group curative effect is better than that of control group （ P〈0.05 ） . [ Conclusion ] Four including yellow honey cream apply umbilical treatment of liver cirrhosis spontaneous peritonitis effect is remarkable, is worth promoting.
