银质针联合导热巡检仪治疗髌下脂肪垫损伤随机平行对照研究

摘　　要：[目的]观察银质针联合导热巡检仪治疗髌下脂肪垫损伤疗效。[方法]使用随机平行对照方法,将80例住院及门诊患者按病志号抽签简单随机分为两组。髌骨下段群刺,患者仰卧于床上,患膝屈曲,膝下垫一软枕,膝部保持130°,在髌骨下1/2段龙胆紫标记针距1cm和向上形成弧形的16∽20个进针点群（因人而异）。局部皮肤常规消毒,0.125%利多卡因皮内注射,形成直径为1cm皮丘,10g/L利多卡因注射液于进针点处进针,选用直径为1.1mm,长13cm银质针沿髌骨下1/2边缘针尾呈扇形刺入到髌尖粗糙面髌下脂肪垫附着处,勿刺入关节腔,留针20min;1周后避开上次针眼,同法进行第2次治疗。治疗组40例电热针髌骨下段群刺,银针刺入后,YRX-1B巡检仪加热20min（温度110℃）,保持银针所接触皮肤表温45℃,加热巡检仪断电,待针身冷却后起针,按压针眼数分钟,碘伏消毒,针眼处皮肤无菌敷料覆盖,三天内勿接触水及不洁物,以防感染。对照组40例艾灸针髌骨下段群刺,银针刺入后,针尾装2cm×1.5cm艾灸一柱,分别滴注95%酒精后点燃,待艾灸燃尽,银质针冷却后拔出,按压针眼,消毒。连续治疗2次（1次/周）为1疗程。观测临床症状、VAS评分、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组治愈15例,显效16例,有效5例,无效4例,总有效率90.00%。对照组治愈10例,显效9例,有效9例,无效12例,总有效率70.00%。治疗组疗效优于对照组（P=0.044〈0.05）。[结论]温控银针治疗髌下脂肪垫损伤效果显著,值得推广。[ Objective ] Observes warm controls the acupuncture needle treatment bin under fat pad damage curative effect. [ Methods ] Use stochastic parallel comparison method, divides into stochastically 80 example in hospitals and the outpatient service patient according to the stochastic tables law two groups.The treatment group 40 example patients lie supine on the bed, contracts the knee flexure, under the knee a pad soft pillow, causes the knee department maintenance 130 o about flexure position, makes the corresponding needle distance in the whirbone next 1/2 section is lcm and forms the arc 16 ~ 20 to enter the pin center group upwardly （ to be different from person to person ）, dragon bilirubin mark.After the partial skin convention disinfection, 0.125% Lidocaine intracutaneous injection, form the diameter is about the lcm skin earthen mound, inoculation fluid in enters the pin center place with 10g/L the Lidocaine to enter the needle, selects the diameter is 1.1mm, the long 13em silver nature needle assumes fan-shaped along the whirbone next 1/2 edge needle tail pricks adheres to stick cohere to bin the sharp rough surface bin under fat pad place, do not prick the ticulare, after the silver nature needle localization inspects the meter heating with the YRX-1B silver nature needle heating, the instrument hypothesis temperature for l l0℃, maintains skin table which the acupuncture needle contacts warm approximately 45 ℃, heats up 20min.After the treatment had ended, waits the needle body to cool the young or up and coming generation needle, holds back the needle eye several minutes, the idodine bends down the disinfection, the needle eye place skin asepsis surgical dressing cover, after the treatment had finished in three days do not contact Shui Ji unclean, guard against the infection.Above after the control group 40 examples the law localization, each silver nature needle needle tail installs a 2cm x 1.5cm Moxa column, after drops the note 95% ethyl alcohol to light separately, waits Moxa to burn through,
