摘  要:目的 探讨超小超顺磁性氧化铁(USPIO)增强磁共振血管成像(MRA)在体无创性评价过氧化物酶体增殖物激活受体γ(PPARγ)激动剂罗格列酮抗动脉粥样硬化的作用。方法 新西兰纯种雄性大白兔25只分为罗格列酮组10只(高脂饲养14周后灌注罗格列酮0.5mg/kg 4周,1次/d)、高脂饲养组10只(高脂喂养),对照组5只(普食18周)。分别在14、18周末,耳缘静脉采血5mL,测定白细胞介素(IL)-6、IL-10水平变化。给予磁共振平扫,USPIO增强72h扫描。计算增强前后T2WI的平均信号强度百分比(SI%)[SI%=|(SIpost-SIpre)|/SIpre×100%],人为规定0~5%为1类强化,6%~20%为2类强化,≥20%为3类强化方式,比较不同强化方式构成比的差异。最后一次扫描后处死动物,解剖血管行病理分析。结果 14周时罗格列酮组、高脂饲养组IL-6水平明显高于对照组(P〈0.01),IL-10比较各组间差异无统计学意义(P〉0.05);但18周末高脂饲养组IL-6水平高于14周[(135.7±25.7)比(106.5±15.9)ng/L,P〈0.01],而罗格列酮组IL-6水平低于14周[(57.3±16.7)比(104.0±23.4)ng/L,P〈0.01]。14周末罗格列酮组与高脂饲养之间MRA强化类型构成比差异无统计学意义(P〉0.05),18周末两组之间强化信号构成比差异有统计学意义(P〈0.01)。3类强化类型图像有124幅,2类强化类型155幅,1类强化类型175幅。病理切片中不稳定斑块131张,稳定斑块133张,正常190张。两者对血管动脉粥样硬化病变的诊断有很好的一致性(κ=0.77,P〈0.01)。结论 罗格列酮稳定斑块的作用可能与降低兔IL-6、稳定IL-10有关。USPIO增强MRA的不同强化类型可以鉴别动脉粥样斑块的易损性。Objective To investigate the feasibility of ultra-small superparamagnetic iron oxide(USPIO)-enhanced magnetic resonance angiography(MRA) in noninvasively evaluating the antiatherosclerotic effect of peroxisome proliferators-activated receptor 7(PPART) agonistrosiglitazone in vivo. Methods A total of 25 healthy male rabbits were randomly divided into rosiglitazone group (n= 10, fed on highfat diet for 14 weeks followed by rosiglitazone 0.5 mg/kg daily for 4 weeks), high fat diet group (n= 10, exposure to high fat diet), and control group (n= 5, treated with normal diet). In the 14th and 18th weeks, 5 mL blood samples were collected from ear vein to detect the changes of interleukin(IL)-6 and IL-10 in serum. Seventy-two-hour USPIO-enhanced MRA was conducted to calculate the percentage of average signal intensity as follows: SI% (SIpost-SIpre) 丨/SIpre× 100%. Type 1 en hancement was defined as SI% 0—5 %, type 2 6 %—20%, and type 3 more than 20 0%. The constituent ratios of different enhancement type were compared. Subsequently, all rabbits were sacrificed after the last scanning and their aortas were dissected for pathology examination. Results In 14th week, the level of IL-6 was markedly higher in rosiglitazone and highfat diet group than that in control group (P〈0.01), while the differences of IL-10 contents among groups were not significant (P〈0.05). In high-fat diet group, the IL-6 concentration in 18th week increased significantly compared to that in 14,h week [(135.7±25.7) vs (106.5±15.9)ng/L, P〈0.01]. However, in rosiglitazone group, the level of IL-6 in 18th week reduced obviously relative to that in 14,h week [(57.3 ±16.7) vs (104.0± 23.4)ng/L, P〈0.01]. There was significant difference in constituent ratio of enhancement types between rosiglitazone group and high-fat diet group in 18th week (P〈0.01), whereas it reached no statistic significance in 14th week (P〉0.05).
