超声造影技术诊断早期乳腺癌病变的应用价值

摘　　要：目的评价超声造影在诊断早期乳腺癌中的应用价值。方法回顾性分析33个乳腺导管原位癌及28个乳腺导管增生(23个普通型导管增生、5个非典型导管增生)的超声造影征象、造影时间-强度曲线定量参数及造影诊断正确率。结果导管原位癌组与导管增生组病灶超声造影后在形态、边界、大小、周围扭曲穿入血管结构、造影剂分布及造影达峰时间、峰值强度方面差异均有统计学意义(P<0.05),造影诊断正确率分别为87.9%(29/33)和71.4%(20/28)。结论早期乳腺癌病变具有较典型的造影恶性特征,超声造影是提高乳腺癌早期诊断正确率的一个有效手段。Objective To evaluate the value of contrast-enhanced ultrasonography（CEUS） in the diagnosis of early breast cancer. Methods The CEUS characteristics including the morphologic features, quantitative parameters of the time-intensity curve in 33 lesions of breast ductal carcinoma in situ（DCIS） and 28 lesions of breast ductal hyperplasia（23 usual ductal hyperplasia and 5 atypical ductal hyperplasia） were retrospectively analyzed. The diagnostic accuracy was compared between the two groups. Results Statistical differences were found in the aspects of lesion shape, margin, size, penetrating or tortuous surrounding vessels, contrast medium distribution, and parameters of time-intensity curve including time to peak（TTP） and peak intensity（PI）（P〈0.05）. The diagnostic accuracies of CEUS in DCIS and breast ductal hyperplasia groups were 87.9%（29/33）and 71.4%（20/28）respectively. Conclusion Early malignant breast lesions have typical CEUS characteristics. CEUS is effective in improving the diagnostic accuracy for early breast lesions.
