布比卡因腰麻剖宫产术中舒芬太尼的半数有效剂量

摘　　要：目的：采用序贯法测定腰麻剖宫产术舒芬太尼的半数有效剂量（ED50）。方法：40例剖宫产患者，选择L3／4间隙穿刺，药物为布比卡因、舒芬太尼的等比重混合液，其中布比卡因固定为7．5mg，初始舒芬太尼剂量为1．2μg，按等比1．2倍的阶梯增减给药，若麻醉平面维持在T6达1h表示成功，则下一例患者梯减一级剂量，反之，下一例患者梯增一级剂量。采用Dixon-Mas-sey计算ED50。结果：21例患者麻醉成功，舒芬太尼的半数有效剂量是0．84μg，95％可信区间是0．78～0．90μg。结论：腰麻剖宫产术中布比卡因7．5mg复合舒芬太尼的半数有效剂量是0．84μg。AIM： To study the minimal effective dose of sufentanil spinal anesthesia in cesarean section. METHODS： Forty patients under- going cesarean section were enrolled in this study and L3/4 was chosen for lumbar puncture. The solution used for spinal anesthesia was sufentanil and bupivacaine, in which the dose of bupivacaine was fixed 7.5μg. While the initial dose of sufentanil was 1.2 μg,according to 1.2 times increase or decrease the administration. If the level of spinal anesthesia reached to T6 and up to 1 hour, the dose decreased 1 grade for next patient. On the contrary, the dose increased 1grade in the next one. The ED50 of sufentanil was calculated by Dixon-Massey sequential method. RESULTS.Twenty-one patients had the good effect of spinal anesthesia. The ED50 of sufentanil for cesarean section was 0.84 μg, and its 95% confidence interval was 0.78--0.90 μg. CONCLUSION. The minimal effective dose of sufentanil combined with bupivacaine 7.5 mg for spinal anesthesia in＇cesarean section was 0.84μg.
