性自慰导致特殊膀胱异物4例报道

摘　　要：目的:探讨性自慰导致特殊膀胱异物的个性化治疗。方法:2011年4月至2013年1月,我院共收治了4例特殊性自慰膀胱异物,根据膀胱异物的种类进行个性化治疗,1例男性患者自行将直径约6mm、33粒磁性钢珠从尿道口塞进尿道后,钢珠坠入膀胱后自觉下腹坠胀,轻微尿痛,无尿频、尿急、排尿中断,我们自行设计前端镶嵌一磁珠的长塑料导管,将其经膀胱镜探入膀胱,吸引出全部钢珠;3例女性自行插入体温计、塑料管、毛笔杆,插入体温计者行膀胱镜下异物取出,余2例形成巨大膀胱结石,均由开放手术取出,其中1例患者病程长达12年,入院时诊断为双侧肾盂、输尿管重度度积水,双侧输尿管下段狭窄,膀胱结石,膀胱周围脓肿,急性肾盂肾炎,给予双侧肾盂穿刺造瘘术,控制感染后行膀胱切开取石术,后将结石电锯开后见其核心为毛笔杆。结果:4例患者均将膀胱异物全部取出,无明显膀胱功能障碍。结论:应仔细询问病史、分析病史,以免患者隐瞒病史而贻误治疗,根据膀胱异物的种类进行个性化治疗,最大限度减少患者的创伤,可自行设计特殊器械;尽量争取经膀胱镜下取出异物。Objectives： To investigate the personalized treatment of special bladder foreign body caused by masturbation. Methods： Four patients with special bladder foreign body cased by masturbation in our hospital from April 2011 to January 2013 received personalized treatment according to the type of bladder foreign body. As regards to the male patient that had 33 magnetic steel balls fallen into the bladder, a long plastic catheter beads via cystoscopy was used to take out all the steel balls. For the three female patients with self - inserted thermometer, plastic tube and pen, the thermometer was removed by bladder endoscope, while huge bladder stones cased by plasLic tube and pen of the other two cases were removed by open surgery. Results： The bladder foreign bodies caused by mas- turbation of the four patients were all removed, without significant bladder dysfunction. Conclusions ： History taking should be conducted in detail to avoid delaying treatment, and personalized treatment should be conducted according to the type of bladder foreign body to reduce maximum trauma.
