无症状胆囊结石预后及影响因素的回顾性研究

摘　　要：目的对无症状胆囊结石患者进行随访,观察预后情况并对影响因素进行分析。方法回顾性收集2007年1月－2013年2月中国民航飞行学院医院确诊的无症状胆囊结石病236例作为研究对象,对无症状胆囊结石病人进行B超复查和对影响转归（出现临床症状或并发症）的因素进行随访问卷调查,2012年2月完成随访工作。对影响预后的因素进行单因素和多元logistic回归分析。结果 236例无症状胆囊结石病例中,随访时间最长72个月,预防性胆囊切除14例（5.9%）,失访18例（7.6%）,最终纳入分析204例,其中未出现临床症状者175例（85.8%）,出现临床症状者29例（14.2%）。单因素及多元logistic回归分析显示：高血脂、糖尿病、酗酒史、超重或肥胖症与无症状胆囊结石预后出现临床症状明显相关（P〈0.05）。结论高血脂、糖尿病、酗酒、超重或肥胖症是无症状胆囊结石出现临床症状和并发症的主要影响因素。Objective To observe the prognosis and to analyze the influential factors of asymptomatic cholecystolithiasis patients through follow-up. Methods There were 236 asymptomatic cholecystolithiasis cases were retrospectively collected in the Hospital of Civil Aviation Flight University of China from January 2006 to February 2012. They were asked to undergo B-Ultrasound examination and answered the questionaire survey of factors affecting the clinical outcomes（ Such as the appearance of the clinical symptoms or complicating disease）. The follow-up was finished in February 2012. The influential factors were analyzed by monovariant and multivariant logistic regression. Results 236 continuous cases were collected： 14（ 5. 9%） cases were undergoed preventive cholecystectomy and 18（ 7. 6%） cases had missing data. The follow-up time was up to 72 months,Thus,the remaining 204 patients were analysized. There were 175（ 85.8%） cases had no appearance of the clinical symptoms,but 29（ 14.2%） cases did. Monovariant and multivariant logistic regression analyses showed that the outcome was associated with hyperlipemia,diabetes,alcohol consumption and overweight or adiposis. Conclusions Hyperlipemia,diabetes,alcohol consumption and overweight or adiposis were influential factors of appearance of the clinical symptoms and complicating disease of asymptomatic cholecystolithiasis.
