Correlation Analysis between Carotid Artery Diseases and Retinopathy in Type 2 Diabetic Mellitus Patients by Ultrasonography

摘  要:目的应用高频超声技术研究2型糖尿病颈动脉病变与视网膜微血管病变的相关性,探讨速度向量成像(VVI)技术评价不同阶段2型糖尿病患者血管弹性差异的临床价值。方法根据检眼镜及眼底荧光造影结果,将96例2型糖尿病患者分为糖尿病无视网膜病变(NDR)组(32例)、非增殖型糖尿病视网膜病变(NPDR)组(28例)和增殖型糖尿病视网膜病变(PDR)组(36例),并与健康体检者构成的对照组(NC)组(30例)进行比较。高频超声采集颈动脉图像,导入VVI脱机软件,测量颈动脉收缩期最大运动速度(Vs)、最大应变(Smax)、最大应变率(SRmax),通过VVI技术分析参数及其曲线变化。结果 NC组、NDR组、NPDR组和PDR组对应点间的VVI技术参数(Vs、Smax、SRmax)比较均有统计学差异(P<0.05),呈递减趋势。NC组Vs(23.61±4.33)×10-2cm/s>NDR组Vs(18.40±4.44)×10-2cm/s>NPDR组Vs(15.54±4.49)×10-2cm/s>PDR组Vs(10.87±3.43)×10-2cm/s;NC组Smax(4.07±0.62)%>NDR组Smax(3.42±0.70)%>NPDR组Smax(2.91±0.40)%>PDR组Smax(2.34±0.61)%;NC组SRmax(0.37±0.06)s-1>NDR组SRmax(0.33±0.07)s-1>NPDR组SRmax(0.28±0.05)s-1>PDR组SRmax(0.22±0.06)s-1。结论 2型糖尿病颈动脉病变与视网膜微血管病变具有相关性,VVI技术能反映2型糖尿病患者血管弹性变化。Objective To investigate the relationship between cornrnon carotid artery (CCA) and retinopathy diseases of type 2 diabetes mellitus (T2DM) patients by high frequency ultrasonography,and discuss the clinical value of velocity vector imaging (VVI) in assessing the elasticity difference of vessel in T2DM patients at different stages.Methods Based on funduscopic examination and fundus tluorescein angiography (FFA),96 cases of type 2 diabetic mellitus patients were divided into three different groups:no diabetic retinopathy (NDR) group (n =32),non-proliferative diabetic retinopathy (NPDR) group (n =28),and proliferative diabetic retinopathy (PDR) group (n =36).All groups were compared with normal control group (NC) formed by 30 healthy individuals.Photographic recordings were collected by the normal high frequency ultrasonography and input into VVI automatic analysis software,maximtum velocity of systole (Vs),maximum of strain (Smax) and maximum of strain rate (SRmax) of the CCA were measured.Then the data revealed by VVI were used to analyze the significance of the changes of curves and indicators.Results Compared with NC group,the parameters of VVI,including Vs,Smax and SRmax,of corresponding points of NDR group,PDR group and NPDR group were gradual declined (P 〈 0.05).V s:NC group (23.61 ±4.33) × 10^-2 cm/s 〉 NDR group (18.40±4.44) × 10^-2 cm/s 〉 NPDR group (15.54±4.49) × 10^-2 cm/s 〉 PDR group (10.87±3.43) × 10^-2 cm/s; Smax:NC group (4.07±0.62)% 〉 NDR group (3.42±0.70)% 〉 NPDR group (2.91±0.40)% 〉 PDR group (2.34±0.61) % ; SRmax:NC group (0.37±0.06) s^-1 〉 NDR group (0.33±0.07) s^-1 〉 NPDR group (0.28±0.05) s^-1 〉 PDR group (0.22± 0.06) s^-1.Conclusion Certain correlation was observed between common carotid artery diseases and retinopathy in type 2 diabetic mellitus patients,and VVI technique can be used for assessing the elasticity difference of vessel of T2DM p
