不同手术配合对胫骨平台骨折的影响分析

摘　　要：目的研究33例胫骨平台骨折手术的手术配合。方法选取收治的33例胫骨远端骨折患者,根据手术配合方式将其分为两组,观察组16例、对照组17例。观察组术前对患者进行心理干预,避免负面情绪对手术产生影响,术后5周进行简单的负重训练。对照组则仅进行常规手术,术后卧床静养。对术前准备、术中配合、器械的清洁保养,及术后护理进行对比。结果 33例胫骨平台骨折患者手术均顺利完成,术后随访5-10个月。观察组术后5周由护理人员陪同进行负重训练,对照组无需进行负重训练,卧床静养。术后两组患者需定期前往医院拍片复查,最终两组患者均无深部感染及切口感染情况,观察组骨性愈合时间9.7±1.2周,较对照组骨性愈合时间10.7±1.5周更快,差异均有统计学意义（均P〈0.05）。观察组满意率为93.75%（15/16）,远高于对照组64.71%（11/17）,差异均有统计学意义（均P〈0.05）。结论通过对比,观察组的手术配合方案更有益于患者的恢复,患者满意度更高。Objective To analyze the condition of operative cooperation for the tibial plateau fracture surgery of 33cases. Methods A total of 33 patients with distal tibial fracture admitted to our hospital were divided into two groups according to the scheme of operative cooperation,with 16 cases in the observation group and 17 cases in the control group.For patients of the observation group,psychological intervention was performed before operation to avoid the influence of negative emotion on the operation,and simple weight training was performed 5 weeks after operation. Patients of the control group received routine operation,followed by bed rest. Preoperative preparation,intraoperative cooperation,maintenance of instruments and postoperative nursing were compared. Results All the 33 patients with tibial plateau fracture received successful surgery and 5 - 10 months of postoperative follow-up. The observation group received weight training accompanied by nurses 5 weeks after operation,while the control group had bed rest instead of weight training. After operation,patients of two groups came back to hospital regularly for X-ray examination. Finally neither deep infection nor wound infection were observed in patients of two groups. The bone healing time of the observation group was 9. 7 ±1. 2 weeks,faster than 10. 7 ± 1. 5 weeks of the control group,and the difference was statistically significant（ P〈 0.05）. The satisfaction rate of the observation group was 93. 75%（ 15 /16）,higher than 64. 71%（ 11 /17） of the control group,and the difference was statistically significant（ P〈 0.05）. Conclusions It can be found through comparison that the operative cooperation scheme of the observation group is more beneficial to the recovery of patients,leading to higher patient satisfaction rate.
