中医药分期联合西药治疗真菌性角膜溃疡随机平行对照研究

摘　　要：[目的]观察中医药分期联合西药治疗真菌性角膜溃疡疗效。[方法]使用随机平行对照方法,将55例住院患者按病志号抽签简单随机分为两组。对照组27例氟康唑滴眼液与那他霉素滴眼液交替点眼,1次/h;托吡卡胺滴眼液,3次/d;妥布霉素滴眼液冲洗结膜囊,6次/d;维生素C,50mg/次,3次/d。治疗组28例早期龙胆泻肝汤(龙胆草12g,栀子9g,黄芩6g,柴胡12g,车前子10g,生地黄12g,泽泻、夏枯草、茯苓、厚朴各9g,甘草6g);中晚期托里消毒散(黄芪20g,当归、白芍、川芎、党参、白术、茯苓各10g,甘草6g,金银花15g,连翘、蝉蜕、白蒺藜各10g);水煎200mL,1剂/d,早晚口服;西药治疗同对照组。连续治疗20d为1疗程。观测临床症状、角膜、前房、视力、真菌菌丝、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈11例,有效14例,无效3例,总有效率89.29%。对照组痊愈4例,有效14例,无效9例,总有效率66.67%。治疗组疗效优于对照组(P<0.05)。[结论]中医药分期联合西药治疗真菌性角膜溃疡效果显著,值得推广。[Objective ] The curative effect observation of combining traditional Chinese and western medicine treatment of fungal corneal ulcer. [ Methods ] Using random parallel control method, 55 cases of hospitalized patients into two groups according to random number table method. The control group of 27 cases of fluconazole eye drops alternating with the doxycycline eye drops his eyes, 1 times/h. Pyrazole card amine eye drops, 3 times/d. Tobramycin eye drops flushing conjunctival sac, 6 times/d. Vitamin C, 50 mg/time, 3 times/d. Treatment group 28 cases early gentian liver soup （ Longdancao 12 g, Zhizi 9g, Huangqi 6g, Chaihu 12g, Cheqianzi 10 g, Shengdihuang 12g, Zexie, Xiakucao, Fuling, Houpu each 9g, Gancao 6g）;Middle-late torrey disinfection powder （ Huangqi 20 g, Danggui, Chuanqiong, Dangshen, Baizhu, Fuling, each 10 g, Gancao 6 g, Jinyinhua 15g, Lianqiao, Chantui, Baijili every 10g ） ;Boiling water 200 ml, 1 / d, oral sooner or later. Western medicine treatment with the control group. Continuous treatment of 20 d 1 course of treatment. Observation of clinical symptoms and adverse reactions. As a course treatment, judge curative effect. [ result ] in treatment group recovered 11 cases, 14 cases effective, 3 had no effect, the total effective rate was 89.29%. The control group recovered in 4 cases, 14 cases effectively, 9 no effect, the total effective rate was 66.67%. The treatment group curative effect is better than that of control group （ P〈0.05 ） . [ Conclusion ] Combining traditional Chinese and western medicine treatment of fungal corneal ulcer effect is remarkable, is worth promoting.
