双向转诊的风险控制研究概述

摘　　要：本研究是在新一轮医改突出"重心下移、关口前移、分层就医"的背景下,在优化卫生资源配置、发挥各级医疗机构特定功能以改变"看病难、看病贵"的需求下,以综合医院与社区卫生服务中心开展双向转诊的实际现状为研究对象,应用风险控制研究的方法,围绕合理有效的患者评价和转送标准的构建和应用,分析综合医院与社区卫生服务中心开展双向转诊可能存在的风险因素,主要包括:缺少合理的转诊标准和程序,转诊的权责界定和分配,利益分配和补偿措施有限以及配套制度尚待完善等4个方面。并提出相应的控制策略和措施,包括:制定双向转诊执行指南和流程,完善补偿和利益分配,明确双向转诊权责分配,完善双向转诊政策制度以及增进医疗机构间沟通,为合理高效地开展双向转诊工作提供参考和帮助。This study is to optimize the allocation of health resources,to play a specific function of the medical institutions at all levels to change in the context of a new round of medical reform highlighting the＂lower the focus of health service,managing the risks well before problems occur and stratified for medical treatment based on Patient＇s condition＂which in helping satisfying the demand＂difficult and expensive＂to see a doctor. Our study object is the current status of two-way referral system in general hospitals and community health service centers,using the method of application of risk control,built around reasonable and effective patient evaluation and transfer standard and analysis of general hospitals and community health service centers risk factors may exist in carrying out two-way referral,including lacking of contrition and standards for two-way referral,clarifying the rights and liabilities for patients and physicians,the distribution of interests and compensation measures are limited,and some policies still need improving. And proposing appropriate control strategies and measures,these are announcing the guides and routines,bettering definite the rights and liabilities,improving the distribution of interests and compensation measures and related policies in which to help provide a reference for the rational and efficient of two-way referral system.
