主成分分析法在医院医疗质量综合评价中的应用

摘　　要：目的：利用主成分分析法综合评价医院医疗质量状况，为提高医院工作质量和管理水平提供科学决策依据。方法选取具有代表性的7个评价指标门诊量（x 1）、出院人数（x2）、手术量（x3）、病床使用率（x4）、病床周转次数（x5）、出院者平均住院日（x6）、病死率（x7），利用软件SPSS13．0对我院2003年至2013年的医疗质量进行主成分分析。结果通过主成分分析得到指标标准值与综合值的回归方程为：y＝0．959x′1＋0．971x′2＋0．964x ′3＋0．8x ′4＋0．908x ′5-0．756x ′6-0．964x ′7。将各指标标准值代入回归方程，可知医院2003年至2013年综合值分别为-9．119、-6．973、-3．144、-2．455、-1．958、0．551、-0．993、1．849、4．893、8．148、9．203，表明我院的医疗质量整体呈上升趋势，但期间2009年低于2008年。结论主成分分析法对医院近11年医疗质量的评价结果，可以较准确直观地反映各年医疗质量的相对情况，有助于医院管理者全面了解医院医疗质量情况，把握医院发展方向，采取对策，提高医院工作质量和管理水平。Objective To evaluate hospital medical quality with principal component analysis , and to provide a scientif-ic decision-making basis for hospital quality and management .Meth ods Seven representative indicators were selected to evalu-ate the hospital medical quality from 2003 to 2013 by principal component analysis with SPSS13.0 which were the outpatient number, the discharged patient number, surgical cases, the bed utilization rate, the bed turnover times, average hospitalization days , and mortality rate.Results The principal component regression equation was y=0.959x′1＋0.971x′2＋0.964x′3＋0.8x′4＋0.908x′5-0.756x′6-0.964x′7.The result showed that comprehensive values in our hospital from 2003 to 2013 were -9.119,-6.973, -3.144,-2.445,-1.958, 0.551, 1.849, 4.893, 8.148, and 9.203, showing an upward trend in our hospital med-ical quality as a whole, except a slight decline in 2009.Conclusion The comprehensive evaluation results of medical quality of our hospital with the use of principal component analysis can directly and accurately reflect hospital performance, and help hospi-tal managers to understand hospital medical quality, grasp the development direction of the hospital, and improve hospital quality and management level.
