摘 要:目的探讨心源性猝死者窦房结组织SCN5A和ANK2表达及其法医学意义。方法收集尸检心脏标本26例,其中心源性猝死者12例(猝死组),排除心脏原因相关的异常死亡者14例(对照组)。应用免疫组化方法检测两组标本窦房结组织中SCN5A和ANK2编码的心肌钠离子通道(Nav1.5通道)和锚定蛋白B(ankyrin-B)的表达。结果猝死组窦房结组织中Nav1.5通道和ankyrin-B的表达均较对照组显著下降,差异有统计学意义(Z=-2.166、-2.505,P<0.05),但在猝死组中两者的表达无明显相关关系(r=-0.337,P>0.05)。结论窦房结组织SCN5A和ANK2表达的改变与心源性猝死的发生相关。Objective To investigate the expressions of SCN5Aand ANK2in sinus nodes of sudden cardiac death(SCD)and their significance in forensic medicine. Methods Twenty-six autopsy heart specimens were collected in this study.Of which,12were SCD,and 14were unnatural deaths of nothing to do with cardiac causes(control group).The expressions of myocardial sodium channel protein(Nav1.5)and ankyrin-B which were encoded by SCN5Aand ANK2in sinus node were detected by immunohistochemical staining. Results The expressions of Nav1.5and ankyrin-B in the sinus node of the SCD were significantly lower than that of the control group(Z=-2.166,-2.505;P〈0.05),and there was no significant correlation between the two expressions in the SCD group(r=-0.337,P〉0.05). Conclusion The changes of the expressions of SCN5Aand ANK2in sinoatrial nodes are correlated with sudden cardiac death.
