孕早期补充叶酸与先兆子痫发生危险性的关系

摘　　要：目的 探讨孕早期服用叶酸与随后先兆子痫发生危险性的关系.方法 选取2012年10月-2013年5月在渭南市妇幼保健院就诊单胎先兆子痫患者69例,按照孕早期是否服用叶酸分为药物干预组（36例）及未干预组（33例）,于孕32-35周检测其血清叶酸含量.结果 ①通过比较药物干预组与未干预组患者进食含有叶酸的食物（绿叶蔬菜、水果、谷类食物、奶、肉、蛋等）对叶酸值检测的影响,结果表明两组叶酸差异无明显统计学意义（9.75 ng/ml vs 9.68 ng/ml,t=0.54,P=0.37）.②采用Logistic回归分析,调整了体重指数、年龄、饮食习惯、教育程度等混合因素后,服用叶酸对预防先兆子痫发生的危险因子OR=1.17（95％置信区间1.08-1.33）.结论 孕早期口服叶酸不能降低先兆子痫发生的危险性.Objective To evaluate the association of folic acid supplementation in early trimester and risk of developing pre eclampsia.Methods The study was conducted on 69 singleton preeclamptic pregnancies deliverd between 32-35 weeks ges tation at Maternal and Child Health Hospital of Weinan City from October 2012 to May 2013.According to the preeclamptic pregnancies if consumed folic acid supplements during the first trimester of pregnancy,69 cases were divided,into two groups：Supplementation of folic acid groups 36 cases and No Supplementation of folic acid groups 33 cases.The serum folic acid concentration were determind.Results ①Comparing the mean levels of food consumption containing folic aicd （vegeta bles,fruits,grains,milk,meat and eggs）indicated that serum folic acid were not significantly statistical different with that of the two groups （9.75 ng/ml vs 9.68 ng/ml,t=0.54,P=0.37.②A logistic regression model analysis was used to adjust for the effects of the main confounders：including body mass index,age,diet,education.The adjusted risk ratio associated with folic acid use was 1.17 （95％ confidence interval,1.08-1.33） in preeclampsia.Conclusion The finding suggests that folic acid supplementation during the first trimester pregnancy do not reduce the risk of preeclampsia.
