高压氧对心肺复苏后大鼠脑保护作用的研究

摘  要:目的:探讨高压氧(hyperbaric oxygen,HBO)治疗对心肺复苏后大鼠的脑保护作用机制。方法:成年Wistar大鼠39只,随机分为假手术组、对照组和HBO组,每组各13只。建立大鼠心肺复苏模型,HBO组大鼠给予HBO治疗。分别于自主循环恢复(ROSC)后6、12、24、42h取血检测S100B蛋白和神经元特异性烯醇酶(NSE)的水平,并观察大鼠大脑皮层组织病理学变化。结果:对照组ROSC后6h、12hS100B水平无差异,ROSC12h以后,S100B水平逐渐下降(P<0.05)。随着时间延长,HBO组S100B水平逐渐下降(P<0.05)。对照组NSE水平随着时间延长逐渐升高(P<0.05)。HBO组ROSC后24h内NSE水平逐渐升高,在ROSC后48h下降。HBO组各时间点S100B和NSE水平均低于假手术组和对照组(P<0.05)。病理学观察示,HBO组大脑皮层组织结构损害程度较对照组轻。结论:高压氧治疗可减轻心肺复苏后大鼠脑损伤,有脑保护作用。Objective:To evaluate the effect of cerebral protection of hyperbaric oxygen(HBO) after cardiopul- monary resuscitation (CPR) in rats. Method:39 adult Wistar rast were randomly divided into 3 groups:Sham operation group,control group and HBO group with 13 cases in each. rat models of CPR were established,and the rats in HBO group were treated by HBO. The level of S100B protein and neurone specific enolase (NSE) were detected at 6 h, 12 h,24 h and 42 h after restoration of spontaneous circulation (ROSC). The pathological changes of cerebral cortex tissue were observed. Result:In control group, there was no significant difference between the levels of S100B at 6 h after ROSC and that at 12 h after ROSC, the levels of S100B decreased gradually after 12 h after ROSC(P〈0.05). With the time prolonged,the levels of S100B in HBO group were declined gradually (P〈0.05), the levels of NSE in control group were increased gradually (P〈0.05). The levels of NSE in HBO group were increased increased gradually within 24 h after ROSC but declined at 48 h after ROSC. At different point of time, the levels of S100B and NSE in HBO group were lower than those in sham operation group and control group (P〈0. 05). Pathological observation showed that the structural lesion of cerebral cortex was minor in HBOgroup than that in control group. Conclusion: HBO can alleviate cerebral injury after CPR,and has the effect of cerebral protection.
