电针预处理对腹腔镜下子宫肌瘤剔除术患者心功能的影响

摘　　要：目的:探讨电针预处理对腹腔镜下子宫肌瘤剔除术患者心肌功能的影响。方法:选取60例行腹腔镜下子宫肌瘤剔除术患者随机分成观察组和对照组,术前对观察组患者内关、列缺、神门穴给予电针预处理连续3天。分别检测术前与术后血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、谷草转氨酶(AST)及乳酸脱氢酶(LDH)的含量,并术中监测各时间点的心率(HR)和平均动脉压(MAP)。结果:对照组在T2及T3时HR及MAP均升高,且术后CK及AST值均高于术前(均P<0.05);而观察组在术中与术后HR、MAP、心肌酶谱与术前均变化不明显(P>0.05),且与对照组相比在T2、T3及术后均明显下降(均P<0.05)。结论:在行腹腔镜下子宫肌瘤剔除术前对患者进行电针预处理,可以有效提高患者血液循环的稳定及保护心肌功能,是一种安全、有效、简便的预防心肌损害的方法,可提高手术麻醉的安全性及降低手术风险。Objective：To study the effect of electroacupuncture pre -treatment on cardiac function of patients with hysteromyomectomy under the laparoscope .Methods：Sixty patients who underwent hysteromyomectomy un-der the laparoscope were randomly divided into observation group （30 cases） and control group （30 cases）. The observation group was given electroacupuncture pre -treatment （ EA on Neiguan , Lieque and Shenmen points）for three days before surgery .The levels of CK, CK-MB, AST, LDH in two groups before anesthesia and 24h post operation were detected.The HR and MAP were monitored and compared .Results：The HR, MAP were significantly increased at T2（5 min after pneumoperitoneum ） and T3（10 min after pneumoperitoneum ）, while the CK , AST were also increased compared with those before pneumoperitoneum in control group （ P 0.05）,which were dramatic declined compared with those in control group at T 2,T3and 24 h post operation time points （ P〈0 .05 ） .Conclusion：Patients with hysteromyomectomy under the laparo-scope who were given electroacupuncture pre -treatment, which could ensure the circulation stability and pro-tect the cardiac functions .It is a new way to prevent the cardiac damage and is safe , effective and convenient , which can improve the safety of operation under anesthesia and reduce the risk of operation .
