摘  要:目的探讨儿童良性中央回一颞区棘波灶癫痫(BECTS)注意缺陷状况及注意网络测试(Attentional networkstest。ANT)检测此类癫痫患者智能障碍中的应用价值。方法收集2009年4月∽2012年12月在海南省人民医院就诊的42例确诊为BECTS的患者为观察组,选择与之相匹配的健康儿童40例作为对照,采用注意网络测试软件评估两组受试者的注意网络状况,比较两组各项测试指标的差异,并使用非条件Logistic分析癫痫波发放的频率、波幅等因素对注意网络指标的影响。结果与对照组比较,观察组ANT的执行控制反应时间较对照组明显延长(P〈0.01);观察组ANT的平均反应时间也较慢(R0.05);两组ANT的警觉和定向反应时间无统计学差异(P〉0.05)。非条件Logistic分析显示,存在高频率或呈高波幅癫痫波发放的受试者ANT的执行功能效率明显下降。结论儿童良性中央一颞区棘波灶癫痫存在注意缺陷,高频率和戚高波幅癫痫波发放可能是导致该组患儿执行智能损害的主要原因之一。ANT在检测BECTSfl,童智能障碍方面有重要应用价值。Objective To investigate the conditions of intelligence function defect in benign epilepsy children with centre-temporal spikes(BECTS) and the value of application of Attention Network Test (ANT) in observation of these epilepsy patients. Methods A total of 42 patients with BECTS (observational group) and 40 healthy children (control group) in Hainan Provincial People' s Hospital were enrolled from April 2009 to December 2012. The ANT was used to investigate the efficiency and compare the difference of test indicators between the two groups. Logistic regression analysis was used to identify the effect of frequency, amplitude of epileptic wave and other factors on the ANT. Results Compared with control group, the executive control network time was significantly longer in patients of BECTS(P〈0.01), the mean reaction time was also lower (P〈0.05); the efficiency of alerting network and orienting network were not statistically different between the two groups (P〉0.05). Logistic regression analysis showed that efficiency of attention executive function decreased significantly in patients with high frequency or high amplitude epileptic wave. Conclusion Attention defect exists in of BECTS children, high frequency and/or high amplitude of epileptic wave may impaire attentional executive network efficiency of the patients. ANT is highly valuable in detectingintelligence defect in BECTS children.
