摘 要:目的观察改良肠内营养输注法在机械通气患者中的效果,以寻求机械通气患者比较安全有效的肠内营养输注方法。方法对入住ICU的93例应用机械通气的危重患者,采用随机数字表法分为2组,实验组的肠内营养采用改良输注方法,对照组应用传统持续输注方法(不间断),分别观察两组患者肠内营养的效果和并发症发生情况,并进行组间比较。结果两组患者胃潴留、反流误吸、吸入性肺炎、胃管堵管发生率的差异均有统计学意义(P<0.05),实验组低于对照组;两组其他消化道症状、代谢并发症、血清白蛋白及疾病预后的差异均无统计学意义(P>0.05)。结论改良肠内营养输注法可以降低机械通气患者肠内营养并发症的发生,安全有效。Objective To evaluate the effects of modified cnteral nutrition(EN) infusion in patients with mechanical ventilation,and to seek the effective and safe infusion method of enteral nutrition. Methods Totally 93 eases of critically ill patients with mechanical ventilation in the intensive care unit(ICU) were randomly divided into two groups. The modified EN infusion method and conventional continuous infusion method(uninterrupted) was imple- mented in the experimental group and the control group,respectively. The outcomes of EN and complications were compared between the two groups. Results The incidences of gastric retention ,food regurgitation and aspiration, aspiration pneumonia and tube blockage in the experimental group were significantly lower than those of the control group(P〈0.05). No significant differences were found in the incidence of metabolic complications,the level of serum albumin and the prognosis of the disease between the two groups(P〉0.05). Conclusion The modified enteral nu- trition infusion method can reduce the complications of enteral nutrition in patients with mechanical ventilation. It has been proved to be safe and effective.
