摘 要:目的:探讨针刀治疗膝骨关节炎可能的生物力学机制。方法:将6个月龄新西兰兔随机分为正常组、模型组、针刀组和针刺组,采用左后肢伸直位固定制动法建立KOA家兔模型。造模后对针刀组与针刺组分别进行治疗干预。各组动物于治疗结束1周后取材,在Bose Electro Force 3300疲劳试验机上对4组试样进行拉伸试验,计算机自动记录数据。结果:模型组的最大应力、最大应变、最大位移和弹性模量与正常组相比均减小(P<0.05);针刀组最大应力、最大应变、弹性模量均大于模型组(P<0.05),最大位移与模型组、空白组之间差异均无统计学意义(P>0.05);针刀组最大应力、弹性模量均大于针刺组(P<0.05,P<0.01),针刀组最大位移显著小于针刺组(P<0.01),针刀组最大应变与针刺组之间差异无统计学意义(P>0.05)。结论:针刀干预能有效恢复内侧副韧带的部分拉伸力学特性,这可能是针刀治疗膝骨关节炎的机制之一。To explore the possible biomechanical mechanism of acupotomy treating knee osteoarthritis (KOA).Methods:Six-months-old New Zealand rabbits were randomly divided into four groups:normal control group,model group,acupotomy group and acupuncture group.The rabbit models of KOA were established by immobilizing the left hind limb in extension position.After modeling, acupotomy and acupuncture were applied to treat rabbits in acupotomy group and acupuncture group respectively.Specimens were acquired one week after treatment.Tensile testing was conducted on Bose Electro Force 3300 protracted test machine and the data was recorded by computer automatically.Results:The maximum stress,maximum strain,maximal displacement and modulus of the model group all decreased compared with normal control group (P〈0.05 ).The maximum stress,maximum strain and modulus of acupotomy group were greater than model group (P〈0.05 ).There was no difference between acupotomy group and model group and normal control group in terms of the maximal displacement (P〉0.05 ).The maximum stress and modulus of acupotomy group were greater than those of the acupuncture group(P〈0.05 ,P〈0.01 ).The maximum displacement of acupotomy group was significantly less than that of the acupuncture group (P〈0.01 ).There was no significant difference of maximum strain between the acupuncture group and acupuncture group.Conclusion:Acupotomology intervention can effectively recover part of the tensile mechanical properties of medial collateral ligament,which may be one of the mechanisms of treating knee osteoarthritis with acupotomy.
