带袢钢板治疗急性肩锁关节脱位的疗效观察

摘　　要：目的 探讨带袢钢板在治疗急性肩锁关节脱位中的临床疗效.方法 对26例RockwoodⅢ以上肩锁关节脱位患者采用带袢钢板（A组）和锁骨钩钢板（B组）治疗,重建喙锁韧带.术后采用Constant评分评估患者功能恢复情况.结果 26例均获随访,时间6～18个月,平均11个月,术后35～60d,肩关节基本恢复正常活动度,术后第1、3个月,A组Constant-Murley评分分别为（89.8±6.2）分、（98.3±2.0）分,B组分别为（84.0±5.8）分、（96.2±2.2）分.术后带袢钢板组的肩关节评分高于锁骨钢板组,差异具有统计学意义（P＜0.05）.结论 带袢钢板重建喙锁韧带是治疗RockwoodⅢ以上肩锁关节脱位的理性方法,复位简单,创伤小,有利于早期功能锻炼,带袢钢板异物反应小,无需取出,避免二次手术的痛苦.Objective To study the linical efficacy of the endobutton in the treatment of acute acromioclavicular joint dislocation. Methods A total of 26 patients with acute acromioclavicular joint dislocation were immobilized with the endobutton （11 cases in group A） or clavicula hook plate （15 cases in group B）. All the patients had the dislocations of or above type Ⅲ according to Rockwood classification. Constant scores were used for postoperative assessments. Results All the patients were followed up, and the duration ranged from 6 months to 18 months（areraged l lmonths）. The motion of the shoulder joint recovered to normal about 35 to 60 days after operation. The scores in group A were 89.8 ± 6.2,98.3±2.0 at the end of lst,3rd month after operation individual. Meanwhile the scores in group B were 84.0±5.8,96.2±2.2. The difference between these two groups was significant（P〈0.05） at the end of lst,3rd month after opera- tion. Conclusion Reconstruction of coracoclavicular ligament by using the endobutton to treat acute acromioclavicular joint dislocation of or above type Ⅲ is a perfect method with advantage of rigid fixation, micro-injury, Small trauma, and early functional exercise.
