摘　　要：目的探讨前列腺突入膀胱程度(IPP)对预测前列腺等离子电切术(TUPKP)术后临床效果的价值。方法收集2007年1月至2012年1月在两家医院接受TUPKP的265例患者的术前临床资料及电切术后6个月的临床资料。我们依据IPP值把患者分为两组:IPP显著组(IPP≥5 mm)105例和IPP非显著组(IPP<5 mm)160例。研究两组手术前后各指标的变化,包括国际前列腺症状评分(IPSS)、生活质量指数评分表(QOL)、前列腺体积(TPV),最大尿流率(Qmax)和残余尿量(PVR),为术后临床效果评估提供参考。结果术前两组各指标无统计学差异。IPP显著组的IPSS术前后变化值和QOL评分术前后变化值与IPP非显著组的相比有明显的升高,且有统计学差异(分别为P=0.038和P=0.000)。但是两组患者在Qmax和PVR的术前后改变值上无统计学差异。结论 IPP可以作为预测TUPKP术后患者IPSS和QOL评分的一个指标。Objective To evaluate the significance of intravesical prostatic protrusion （IPP） in predicting postoperative outcomes in patients with benign prostatic hyperplasia. Methods The parameters of 265 patients with a follow-up of 6 months who were treated with bipolar transurethral plasma kinetic prostatectomy （TUPKP） were analyzed. We divided the patients into two groups on the basis of the degree of IPP： the significant IPP group （IPP ≥ 5 mm, n=105） and the no significant IPP group （IPP 〈 5 mm, n=160）. Preoperative and postoperative changes in parameters, such as the International Prostate Symptom Score （IPSS）, quality-of-life （QOL） score, maximum urinary flow rate （Qmax）, and postvoid residual urine （PVR） were analyzed. Results The preoperative characteristics had no significant difference between two groups. Postoperative changes in IPSS and QOL score were higher in the significant IPP group than in the group with no significant IPP and there were significant differences （P=-0.038 and P=-0.000, respectively）. Changes in Qmax and PVR were not significantly different between the two groups. Conclusion IPP is an independent factor for predicting postoperative outcomes of IPSS and QOL.
