摘 要:目的探讨孕妇血清基质金属蛋白酶-9(MMP-9)水平变化及胎盘组织中MMP-9 mRNA表达变化在子痫前期发病中的作用。方法选取40例重度子痫前期孕妇,按发病时孕周不同分为早发型组(发病孕周≤34w)和晚发型组(发病孕周>34w)各20例;另选20例正常晚期妊娠妇女为对照组。采用RT-PCR技术检测3组孕妇胎盘组织中MMP-9 mRNA表达水平;采用酶联免疫吸附(ELISA)法检测3组孕妇血清MMP-9水平。结果早发型组及晚发型组孕妇血清中MMP-9水平均高于对照组,两两比较,差异均有统计学意义(P<0.05),且早发型组高于晚发型组(P<0.05)。早发型组及晚发型组孕妇胎盘组织中MMP-9 mRNA表达水平均低于对照组,两两比较,差异均有统计学意义(P<0.05),且早发型组低于晚发型组(P<0.05)。结论子痫前期孕妇血清MMP-9水平的上升及在胎盘组织mRNA的表达下降可能通过不同途径参与子痫前期的病理生理过程,可能参与子痫前期的发病。Objective: To investigate the change of matrix metalloproteinase 9 (MMP-9) in maternal serum and the effects of MMP-9 mRNA expression in the pathogenesis of preeclampsia (PE) . Methonds: 40 women with severe PE were divided into early-onset group (n =20, onset time-~〈34 gestational weeks) and late-onset group (n =20, onset time〉34 gestationalweeks) , and 20 healthy pregnant women were as control. Real-time fluorescent quantitative reverse transcription PCR (RT-PCR) was used to detect placental MMP-9 mRNA expression. Enzyme-linked immunosorbent assay (ELISA) was used to detect the maternal serum level of MMP-9.Results: The levels of serum MMP-9 in the early-onset and late-onset severe PE groups were significantly higher than that in the control group (P〈0.01) , and the levels in the early-onset group was higher than that in the late-onset group (P〈0.05) .There was significant statistical difference (P〈0.01 ) . The levels of placental MMP-9 mRNA in the early-onset and late-onset severe PE groups were significantly lower than that in the control group (P〈0.05) , and the levels in the early-onset group was lower than in the late-onset group (P〈0.05) . There was significant statistical difference (P〈0.01) . Conclusions: The Increased levels of serum MMP-9 and decreased placental expressionsof MMP-9 mRNA in PE pregnant women may be involved in the pathophysiological process of preeclampsia through different pathways.They may be involved in the pathogenesis of preeclampsia.
