摘 要:目的探讨湖南急性呼吸道感染患儿中鼻病毒及病原的流行病学分布特征。方法收集急性呼吸道感染患儿1447份鼻咽抽吸物(NPAs)样本及病历资料,用聚合酶链反应方法(PCR)、RT-PCR、巢式PCR方法检测NPAs中鼻病毒(HRv)及其他11种常见呼吸道病毒,同时进行细菌培养、分离及鉴定。结果(1)1447份样本中HRV检出率为18.73%(271/1447),其他呼吸道病毒检出依次为呼吸道合胞病毒(RSV)14.44%、副流感病毒(PIV3)10.85%,博卡病毒(HBoV1)8.15%、腺病毒(ADV)7.12%、偏肺病毒(HMPV)3.59%、流感病毒A、B型(IFVA、IFVB)1.94%和1.52%、副流感病毒1、2型(PIV1、PIV2)0.90%和0.28%、冠状病毒NL63和HKU10.62%和0.55%;(2)HRV检出率在年龄、性别分布差异无统计学意义(P〉0.05),在5月份检出率最高(38.24%);(3)HRV检出阳性与混合感染有相关性(P〈0.00),多因素回归分析发现HRV检出阳性与RSV(OR=O.618,95%C10.405-0.944)、HMPV(OR=0.224,95%C10.069-0.725)、ADV(OR=2.044,95%C11.156-3.612)感染密切相关。结论HRV是引起小儿急性呼吸道感染的主要病毒病原体,HRV与RSV、HMPV、ADV的混合感染可能是呼吸道感染致病的主要原因。Objective To investigate the epidemiological distribution characteristics of human rhinovirus(HRV) and other pathogens with respiratory tract infection in children. Methods Nasopharyngeal aspirate samples(NPAs) of 1 447 cases and clinical data were collected, HRV and other 11 kinds of respiratory viruses were detected by PCR, rt-PCR and nest PCR and bacterias were cultured, separated and identificated with automated microbial analysis system. Results (1) HRV-positive rate was 18.73% (271/1 447), RSV(14.44%), PIV3(10.85%), HBOV1(8.15%), ADV(7.12%), HMPV(3.59%), IFVA, IFVB (1.94%), IFVB(1.52%), PIV1(0.90%), NL63(0.62%) HKUl(0.55%), PIV2(0.28%). (2) Age and gender distribution of HRV detection rate had not been statistically significant (P〉0.05), it was highest in May (38.24%). (3) HRV was frequently to be detected co-infection with other respiratory viruses (P〈0.00), multi-factor analysis showed that HRV-positive was closely related to RSV (OR=0.618, 95% CI 0.405-0.944), HMPV (OR=0.224, 95% C1 0.069-0.725) and ADV (OR=2.044, 95% CI 1.156-3.612). Conclusion HRV was a major respiratory tract infection pathogens in children, co-infection may be the main reason with RSV, HMPV and ADV for pathogenic respiratory infection.
