摘　　要：目的探讨在线血液透析滤过(on-line hemodiafiltration,OLHDF)对维持性血液透析患者微炎症和营养状况的影响。方法选择维持性血液透析的终末期肾病患者40例,随机分为2组:OLHDF组和HD组,每组20例,分别于治疗前和治疗3个月后采血检测血清白蛋白(albumin,Alb)、C-反应蛋白(c-reactiveprotein,CRP)、白介素6(interleukin-6,IL-6)水平。结果 HD组与OLHDF组患者治疗前血CRP、IL-6和Alb差异均无统计学意义(P>0.05)。治疗后,HD组患者CRP和IL-6水平较治疗前无明显改变(P>0.05);OLHDF组治疗后血CRP和IL-6水平较治疗前均明显下降(P<0.05)。HD组和OLHDF组治疗后较治疗前Alb均升高(P<0.05)。OLHDF组治疗后Alb升高幅度较HD组明显(P<0.05),CRP和IL-6水平下降幅度较HD组大(P<0.05)。结论 OLHDF治疗维持性血液透析患者可有效清除中大分子炎症因子,减轻微炎症状态,改善MHD患者的营养状况。Objective To investigate the effect of online hemodiafiltration on micro -inflammation and nutritional statusin maintenance hemodialysis patients .Method s Forty maintenance hemodialysis patients with end-stage renal disease （ESRD） were randomly divided into two groups：OLHDF group and HD group,with 20 patients in each group.The serum levels of albumin（Alb）,C-reactive protein（CRP） and interleukin-6（IL-6） were detected before treatment and 3 months after treatment.Results Before treatment there were no significant differences in the serum levels of Alb ,CRP and IL-6 between HD group and OLHDF group（ P 〉0.05）.After treatment there were no significant differences in the serum levels of CRP and IL-6in HD group,as compared with those before treatment （ P 〉0.05 ）,however,the serum levels of CRP and IL-6 were significantly decreased in OLHDF group,as compared with those before treatment （ P 〈0.05）.After treatment the levels of Alb in HD and OLHDF group were obviously increased ,as compared with those before treatment（ P 〈0.05）,moreover,the increase amplitude of Alb in OLHDF group was more obvious than that in HD group （ P 〈0.05）,however,the decrease amplitude of CRP and IL-6 in OLHDF group was more obvious than that in HD group （ P 〈0.05）.Conclusion OLHDF can effectively clear the median-macromolecules inflammatory factors in maintenance hemodialysis patients ,relieve micro-inflamma-tory state and improve the nutritional status of MHD patients .
