流式细胞仪法与磁性酶联免疫吸附法检测HLA-B27的对比研究

摘　　要：目的:对比分析磁性酶联免疫吸附法与流式细胞仪法检测AS患者HLA-B27的临床价值。方法:选取2010年5月-2013年5月笔者所在医院门诊及住院疑诊为AS患者164例,分别应用磁性酶联免疫吸附法和流式细胞仪法检测HLA-B27,以临床诊断为参照,对比分析两种方法的特异性、敏感性及准确性等指标。结果:164例疑诊为AS患者中,确诊143例,排除21例。以临床诊断为参照标准,流式细胞仪法及磁性酶联免疫法敏感性分别为96.5%、84.6%,两方法敏感性比较差异有统计学意义(字2=11.820,P=0.001);特异性分别为71.4%、95.2%,流式细胞仪法特异性低于磁性酶联免疫吸附法(字2=4.286,P=0.038);准确性分别为93.3%、86.0%,两种方法比较差异有统计学意义(字2=4.725,P=0.030);关于阳性预测值及阴性预测值,两种方法比较差异无统计学意义(字2=2.887,P=0.090;字2=0.625,P=0.419)。结论:磁性酶联免疫吸附法及流式细胞仪法各有优缺点,两者结合对早期AS的诊断有着重要的参考价值。Objective：To analyze the clinical value of flow cytometry method and magnetic enzyme linked immunosorbent assay for detection of HLA-B27 in paitients with ankylosing spondylitis（AS）.Method：164 patients with suspected AS were chencked with the flow cytometry method and magnetic enzymelinked immunosorbent assay for detection of HLA-B27 from May 2010 to May 2013 in our hospital.The clinical diagnosis results being served as a reference,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of two methods were analyzed.Result：143 cases were diagnosed and 21 cases were excluded in 164 cases of suspected AS patients.The clinical diagnosis being severed as the reference standard,the sensitivity of flow cytometry method and magnetic enzyme linked immunosorbent assay were 96.5%,84.6%.The specificity of two methods were 71.4%,95.2%,and the accuracy was 93.3%, 86.0%.There were significant differences between two methods（χ2=11.820,P=0.001;χ2=4.286,P=0.038;χ2=4.725,P=0.030）.About the positive predictive value and negative predictive value of two methods there were no significant differences between two methods （χ2=2.887,P=0.090;χ2=0.625,P=0.419）. Conclusion：Magnetic enzyme linked immunosorbent assay and flow cytometry method have their respective advantages and disadvantages,and two methods together have an important reference value for AS.
