峡部裂性腰椎滑脱相关影响因素对照分析

摘　　要：目的探讨峡部裂性腰椎滑脱与相关影响因素的关系。方法选取我科2010年4月至2013年12月共62例峡部裂性腰椎滑脱患者,其中男性28例,女性34例;对照组选取同期性别、年龄相匹配的病例62例,两组病例发病部位均为L5。对两组病例椎弓峡部宽度、关节突关节面与矢状面夹角、退变程度、腰骶角等因素与腰椎滑脱关系进行分析研究,所得数据应用统计学软件SPSS13.0进行处理,对两组计量资料和计数资料分别应用t检验和χ2检验,得出相应结论。结果应用t检验分别对峡部裂组与对照组下列均数进行比较:(1)椎弓峡部宽度:t=-6.553,P<0.001;(2)关节突关节面与矢状面夹角:t=2.381,P<0.01;(3)腰骶角:t=2.304,P<0.05;以上均数差异均有统计学意义。两组椎间关节退变程度经χ2检验,χ2=16.160,P<0.01,差异有显著统计学意义。结论椎弓峡部发育不良或形态改变(纤细)为峡部裂性腰椎滑脱的重要因素,而椎间关节夹角、退变程度以及腰骶角在峡部裂性腰椎滑脱形成中起到一定作用,或者为其继发改变,两者具有协同作用。Objective To investigate and study the lumber isthmic spondylolisthesis and its related factors. Methods Sixty-two patients with lumbar spondylolisthesis in our department from 2010 April to 2013 December were selected as research object （spondylolisthesis group）, including 28 males and 34 females; the control group from the same gender, age matched 62 cases. L5 is the common site of pathological changes of two groups. Analysis of two cases of vertebral arch isthmus width, facet and sagittal angle, degeneration, lumbosacral angle of lumbar spondylolis- thesis. The data was analyzed by SPSS13.0 softare. T-test was used for two groups of quantitative data and chi square test for count data. The resuce of P〈0.05 was statistically significant. Results T test of spondylolisthesis group and control group were compared： （1） isthmus width showed statistically significant difference between the two groups （t=-6.553, P〈0.001）; （2） facet and sagittal angle showed statistically significant difference between the two groups （t=2.381, P〈0.01）; （3） lumbosacral angle showed statistically significant difference between the two groups （t= 2.304, P〈0.05）. Intervertebral joint degenerationby showed statistically significant difference between two grops （X^2= 16.160, P〈0.01）. Conclusion Vertebral arch isthmus dysplasia or morphological changes （slender） is an important factor in isthmic spondylolisthesis, and intervertebral joint angle, degeneration and lumbosacral angle play a role in the isthmic spondylolisthesis or act as the secondary changes of the isthmic spondylolisthesis, which have a synergistic effect.
