微创稳定系统钢板内固定治疗老年糖尿病患者股骨髁上骨折疗效观察

摘　　要：目的：探讨微创稳定系统（LISS）钢板内固定治疗老年糖尿病患者股骨髁上骨折的疗效。方法选取2004年1月-2012年10月秦皇岛市第一医院收治采用LISS钢板内固定治疗的老年糖尿病患者股骨髁上骨折32例。入院后控制血糖，采用闭合复位LISS钢板内固定治疗股骨髁上骨折，观察手术时间、术中出血量、切口愈合时间及随访膝关节功能（HSS）评分。结果本组患者手术时间60～90 min，平均70 min；术中出血量200～400 ml，平均250 ml；切口愈合时间14～18 d。无切口感染、延迟愈合或不愈合。平均随访14个月。骨折愈合时间为6～12个月，平均10个月。无内固定物松动、断裂。患肢膝关节活动范围均恢复到受伤前水平。随访HSS评分57～91分，平均71.6分，其中优9例，良20例，中2例，差1例。结论闭合复位LISS 钢板内固定治疗老年糖尿病患者股骨髁上骨折，手术创伤小、术后并发症少。ObjectiveTostudythecurativeeffectofclosedreductionandLISSsteelplateinternalfixationinthetreat-ment of elderly patients with diabetes and supracondylar fracture .Methods A total of 32 patients with supracondylar fracture and diabetes who admitted to the First Hospital of Qinhuangdao from January 2004 to October 2012 were selected as study sub-jects.Blood sugar level of patients was controlled after admission , LISS steel plate internal fixation was used to treat supracondylar fracture, duration of operation , bleeding volume in operation , healing time of incision and follow -up record of knee functional （HSS） score were observed.Results The duration of operation time among patients in study group was 60-90 minutes, average duration of operation time was 70 minutes.Bleeding volume in operation was about 200-400 ml, average bleeding volume was 250 ml.Incision healing time was 14-18 days.No incision infection , delayed healing and nonunion cases .The mean followed-up was 14 months.Fracture healing time was 6 -12 months, average healing time was 10 months.No case was found to have looseness or breakage of internal fixation .The range of motion （ ROM） of the knee joint returned to the level of before the inju-ry.Follow-up record of HSS was 57-91 points, average score was 71.6 points, HSS was excellent in 9 cases, good in 20 ca-ses, fair in 2 cases, and poor in 1 case.Conclusion LISS steel plate internal fixation in the treatment of elderly patients with diabetes and supracondylar fracture had the advantages of lesser trauma and lesser postoperative complications .
