摘 要:目的探讨神经节苷脂治疗早产儿脑损伤的疗效。方法选择2010年2月至2012年2月在新乐市医院儿科住院治疗的84例早产脑损伤患儿作为研究对象,根据随机数字表法将患儿分为两组,各42例,两组患儿均采用常规治疗,单唾液酸四己糖神经节苷脂治疗组在常规治疗的基础上采用单唾液酸四己糖神经节苷脂治疗,脑活素治疗组在常规治疗的基础上应用脑活素治疗。比较两组患儿纠正胎龄40周时呼吸暂停发生率、新生儿行为神经测定(NBNA)评分,纠正胎龄3个月和6个月时发育商(DQ)值以及纠正胎龄3个月时的疗效。结果单唾液酸四己糖神经节苷脂治疗组患儿在纠正胎龄40周时呼吸暂停发生率显著低于脑活素治疗组(χ2=8.021,P<0.05),NBNA评分显著优于脑活素治疗组(t=7.9919,P<0.05);单唾液酸四己糖神经节苷脂治疗组患儿纠正胎龄3个月和6个月时DQ值均显著高于脑活素治疗组(t=6.809、7.197,P<0.05);单唾液酸四己糖神经节苷脂治疗组临床疗效优于脑活素治疗组(Z=7.806,P<0.05)。结论神经节苷脂能够促进损伤神经的修复以及神经细胞的再生,其对早产脑损伤患儿疗效确切,对于患儿早期恢复,改善预后具有重要意义。Objective To investigate the clinical effect of brain injmy in preterm infants treated with neurotrophic drugs.Methods Total of 84 preterm infants with brain injury who were treated in pediatrics of Xinle City Hospital from Feb.2010 to Feb.2012 were divided into two groups by random number table method,42 cases were in each group,both groups were treated with routine treatment,the ganglioside group was also treated with ganglioside based on conventional treatment,the cerebrolysin group treated with cerebrolysin based on conventional treatment.The rates of apnea and NBNA scores of preterm infants in two groups at 40 weeks after correcting gestational age were compared,and DQ value of 3 months and 6 months was determined,and the total clinical efficiency of preterm infants at 3 months after correcting gestational age was compared.Results The apnea of ganglioside group at 40 weeks after correcting gestational age was significant lower than that of the cerebrolysin group(x2 =8.0212,P 〈 0.05).NBNA score was better than that of the cerebrolysin group(t =7.9919,P 〈 0.05).DQ value of ganglioside group at 3 months and 6 months after correcting gestational age was higher than that of the cerebrolysin group(t =6.809,7.197,P 〈 0.05).The clinical effect of preterm infants in ganglioside group was better than that of the cerebrolysin group(Z =7.806,P 〈0.05).Conclusion Ganglioside can promote the repair of injured nerve and regeneration of nerve cells,which has the exact curative effect in the treatment of brain injmy in preterm infants,and a great significance for early recovery and improvement of prognosis of the brain injmy of preterm infants.
