点对点精神卫生技术服务社区干预效果分析

摘　　要：目的：探讨精神科医生与社区建立的点对点精神卫生技术支持对社区的干预效果。方法精神科医生进入社区点对点技术服务1年前后，随机抽取新乡市城市、农村各2个社区/乡的49名社区医生和117名社区管理的精神分裂症患者，评估社区医生的精神卫生知识知晓率；调查患者治疗依从性、药物自我管理能力，应用圣路易社区生活技巧评估量表（ SLICLS-C）对患者日常生活情况进行评定。结果干预前（即点对点技术支持前）精神卫生基本知识的回答正确率为10.2%-93.9%，平均为53.1%；干预后的回答正确率为69.4%-100.0%，平均为95.2%；干预前后除在条目4、7、11、12、16上差异无统计学意义外，在其余条目上差异均有统计学意义（ P﹤0.05）。患者的依从性、药物自我管理能力均较干预前提高，差异有统计学意义（ P﹤0.05）；SLICLS-C评分中沟通能力评分较干预前提高，差异有统计学意义（ P﹤0.05）。结论建立精神专科医生与社区技术支持的长效机制，能有效提高社区精神卫生的综合服务能力。Objective To study the effect of point-to-point mental health technology support on community service. Methods One year after psychiatrists entered community to perform point to point technology service,49 community doctors and 117 schizophrenia patients under community management were randomly selected from two communities in Xinxiang：one from rural area,the other from urban area,and evaluated doctors＇ awareness rate of mental health knowledge and patients＇ treatment compliance as well as drug self-management ability. Daily life condition of the patients was assessed by St. Louis community living skills assessment scale（SLICLS-C）. Results Before intervention,the rate of right answers to basic mental health knowledge was 10. 2% -93. 9% with an average of 53. 1%. After intervention,the rate was 69. 4% - 100. 0% with an average of 95. 2%. The differences in other items except items 4,7,11,12 and 16 were significant（P﹤0. 05）. Patients＇ compliance and drug self-management ability all had some improvement after intervention and the difference was significant（ P﹤0. 05 ）. The result of SLICLS - C showed that patients＇ communicating skill improved after intervention with statistical significance （P﹤0. 05）. Conclusion The establishment of long-term mechanism of psychiatric doctors and technique support can effectively improve comprehensive service ability of community mental health.
