右中指近侧指间关节创伤性骨关节炎伴鹅颈指畸形关节置换手术一例

摘　　要：目的探讨Swanson人工关节置换治疗创伤后指间关节僵硬畸形疼痛的临床疗效。方法对右中指近侧指间关节创伤性骨关节炎伴鹅颈指畸形病例一例行人工关节置换术，尺侧侧副韧带重建术，锤状指畸形矫形术。结果一例患者手术顺利，切口一期愈合，半年随访鹅颈指畸形矫正，略锤状指畸形，无关节疼痛，置换的近侧假体关节外观良好，运动功能可，伸屈活动范围60°～80°。根据中华医学会手外科学会上肢部分功能评定试用标准评定，手功能评定：良。结论人工关节假体置换治疗手指近侧指间关节创伤性骨关节炎具有手术操作简便、创伤小、恢复快的优点，但术后功能依据患者要求而定。Objective To study the clinical effect of adopting Swanson artificial joint replacement to treat traumatic interphalangeal joint stiff deformity pain.Methods Artificial joint replacement surgery was conducted on one case of right middle finger proximal interphalangeal traumatic osteoarthfitis with swan-neck fingem deformity together with reconstruction of ulnar collateral ligaments and deformity orthopedics of mallet finger. Results The surgery went smoothly with the primary healing of incision, and half a year follow-up visit showed swan-neck deformity correction, slight mallet finger deformity, no joint pain, fine appearance of replaced proximal prosthetic joint, and fa- vorable movement function with flexion range of 60~ ~ 80~.According to the tentative standard of upper limp partial function evaluation by Hand Surgery Branch of Chinese Medical Association, the hand function evaluation was good.Conclusion Artificial joint prosthesis replace- ment to treat finger proximal interphalangeal traumatic osteoarthritis has the advantages of easy operation, minor trauma, and quick recovery. But the postoperative function should meet the requests of sufferers.
