腹腔镜下联合不同药物治疗重度子宫内膜异位症的临床疗效分析

摘　　要：目的探讨腹腔镜下联合不同药物治疗重度子宫内膜异位症的临床治疗效果。方法选取2012年1月至2013年6月间笔者单位收治的80例重度子宫内膜异位症患者作为本文研究资料;并随机将80例患者分为两组:孕三烯酮组40例,腹腔镜下保守性手术治疗后,口服孕三烯酮胶囊;2.5mg/次,2次/周;GnRH-a组40例,腹腔镜下保守性手术治疗后,皮下注射促性腺激素释放激素(GnRH-a)3.75 mg/(次·4周)。观察两组患者术后6个月时的痛经、非经期下腹痛、性交痛等特征性临床症状的改善情况及不良反应发生情况。结果术后6个月时,GnRH-a组术后总有效率明显高于孕三烯酮组(P<0.05);同时,复发率则明显低于孕三烯酮组(P<0.05);GnRH-a组患者不良反应发生情况也明显好于孕三烯酮组(P<0.05)。结论腹腔镜下联合GnRH-a治疗重度子宫内膜异位症,有效地改善患者临床症状,全面提升治疗总有效率,不良反应发生率低,具有较好的临床推广应用价值。Objective To investigate the treatment of severe combined different endometriosis clinical therapeutic effect of the drug laparoscopie. Methods January 2012 - June 2013 between the author units treated 80 eases of severe endometriosis patients, as this study data; and 80 patients were randomly divided into two groups： Gastronome groups： 40 cases after laparoscopic conservative surgery, oral capsules gastronome; 2.5mg/ times, 2 times / week; GnRH-a group： 40 cases, conservative laparoscopic surgery, subcutaneous gonadotropin- releasing hormone （GnRH-a） 3.75mg / （ times o four weeks ）. Improvement and adverse reactions observed after the two groups of patients at 6 months of dysmenorrheal, non- menstrual pain, painful intercourse and other characteristic clinical symptoms were observed. Results After six months, GnRH-a group were significantly higher than the total efficiency Gastronome group （P 〈 0. 05 ） ; Meanwhile, the recurrence rate was significantly lower than Gastronome group （ P 〈 0. 05 ） ; GnRH-a group of patients with adverse events was significantly better than gastronome group （ P 〈 0. 05 ）. Conclusions Laparoscopic combined with GnRH-a treatment of severe endometriosis, can effectively improve the clinical symptoms, and enhance the overall efficiency of the treatment, a low incidence of adverse reactions, with better clinical application value.
