摘　　要：目的 了解武汉铁路局职工糖尿病视网膜病变（DR）患病率及其危险因素.方法 对2011年9月至2013年5月在汉口医院体检中心体检的20- 75岁武汉铁路局职工（11 873人）为调查对象,进行糖尿病（DM）筛查.根据WH0 1999年DM的诊断标准确诊并进一步行DR筛查：具体包括问卷调查、一般项目检查、实验室检测、眼科检查、眼底照相和荧光素钠造影检查等.分析DM患者中DR的患病率及危险因素.结果 共确诊DM患者380例,DM患病率为3.20％（380/11873）.380例DM患者中发现DR 81例,占21.32％（81/380）.DR患者和无DR患者间DM病程、HbA1C、FPG、TG、LDL、是否使用胰岛素及文化程度比较,差异具有统计学意义（P＜0.05）.DM病程、高HbA1C、高FPG、高TG和高LDL是DR发生的独立危险因素（P＜0.01）.结论 武汉铁路局职工DM患者中DR患病率为21.32％ （81/380）.DM病程、高HbA1C、高FPG、高TG和高LDL是DR发生的独立危险因素.Objective To learn the prevalence and risk factors of diabetic retinopathy （DR） of servants in Wuhan railways bureau,Hubei province.Methods A total of 11873 Wuhan railways bureau servants （20-75 years old） were enrolled in this study.A11 participants diagnosed with diabetes underwent questionnaire,ordinary project examine,laboratory examine,ophthalmologic examination,stereoscopic fundus photography and fundus fluorescein angiography （FFA）.Photographs and FFA were graded in a masked manner to assess the presence and severity of DR and diabetic macular edema （DME）.Results Of 11873 servants,DM was confirmed in 380 patients,the prevalence was 3.20% （380/11873）.Of 380 DM patients,DR was in 81 patients,the DR prevalence was 21.32% （81/380）.There were statistically difference on the duration of diabetes,Glycosylated hemoglobin （HbA 1C）,Fasting plasma glucose （FPG）,Triglyceride （TG）,Low density lipoprotein （LDL）,whether or not to use insulin and levels of education between DR group and non DR group （P 〈0.05）.The duration of diabetes,high HbA 1C,high FPG,high TG and high LDL were the independent risk factors of DR （P 〈0.01）.Conclusions DR prevalence is 21.32% （81/380） in Wuhan railways bureau servants.The duration of diabetes,high HbA 1C,high FPG,high TG and high LDL are the independent risk factors of DR.
