微小切口皮内缝合疝囊高位结扎术治疗小儿腹股沟斜疝的临床疗效

摘　　要：目的观察微小切口皮内缝合疝囊高位结扎术的临床疗效,探讨和分析其术后并发症的原因。方法对我院附院及教学医院自2009年1月至2014年3月收治小儿腹股沟斜疝528例患者的完整临床资料进行回顾性分析,观察近期和远期疗效。结果手术切口平均愈合时间为7.6 d,伤口愈合良好511例(96.8%),伤口愈合欠佳10例(1.9%),切口化脓7例(1.3%),没有出现血肿或渗血、咳嗽、便秘和排尿困难。远期疗效观察治愈504例(95.4%),显效12例(2.3%),复发9例(1.7%),失访3例(0.6%)。结论微小切口皮内缝合疝囊高位结扎术具有切口小,术后愈合快、皮肤伤口愈合美观、手术易操作、术后并发症少等优点,可作为治疗小儿腹股沟斜疝的首选方法。Objective To evaluate the clinical efficacy of high ligation sac with tiny incision and subcutaneous suturation, and the causes of postoperative complications were explored and analyzed. Methods The clinical data of 528 pediatric patients with indirect inguinal hernia were retrospectively reviewed. Pediatric patients admitted to the teaching hospital and affiliated hospital of medical college from January 2009 to March 2014. Shortand long-term efficacy were observed. Results Short-term effects： the average time of incision healing was 7.6 d. Good incision healing occured in 511 cases （96.8%） ;poor incision healing in 10 patients （ 1.9% ） ;Incision suppuration in 7 cases （ 1.3 % ）. No hematoma, bleeding, cough, constipation, dysuria occurred. Long-term effects ： 504 cases （ 95.4% ） cured, 12 cases （ 2.3 % ） functioned,9 cases （ 1.7% ） recurred,3 cases （0.6%） lost. Conclusion The method of high ligation sac with tiny incision and subcutaneous suturation has the advantages of tiny incision, wound healing fast, fewer complications and easy to operate, which can he used as the preferred method for pediatric inguinal hernia.
