摘  要:目的通过检测不同病毒感染急性期患者外周血淋巴细胞亚群,比较其差异,了解不同病毒感染对宿主细胞免疫功能的影响并分析其临床意义。方法收集2010~2013年感染科门诊及住院收治的51例病毒感染急性期患者及20例正常对照,检测其外周血淋巴细胞亚群,比较其差异并分析临床意义。结果与正常对照相比,病毒感染患者外周血NK细胞、B细胞比率减少,T细胞比率升高;T细胞亚群中CD4+CD28+T细胞和CD8+CD28+T细胞比率降低,CD8+T细胞以及CD8+CD38+T细胞比率升高。EB病毒感染后T细胞、CD8+T细胞、CD8+CD38+T细胞比率升高较其他病毒感染更为显著。结论病毒感染患者存在免疫功能紊乱,天然免疫、体液免疫及细胞免疫功能下降同时伴有细胞免疫异常激活,EB病毒感染较其他病毒免疫激活更为明显。Objective To investigate the different changes of hostg cellular immunity in patients with different virus infection by compa- ring peripheral blood lymphocyte subsets. Methods Flow cytometry was used to detect the peripheral blood lymphocyte subsets among 51 patients infected with Epstein - Barr virus, cytomegalovirus or other virus. Results In contrast to the normal controls, percentage of T ceils increased, and percentage of B/NK cells decreased among the patients with acute virus infection. Meanwhile, within T cell subsets, percentage of CD8 + T cells and CD8 + CD38+ T ceils increased ; and percentage of CD4 + CD28 + T ceils and CD8 + CD28 +T ceils decreased. And percentage of T ceils, CD8 + T cells and CD8+ CD38 + T cells was higher among the patients infected with Epstein - Barr virus than that among the patients infected with cytomegalovirus or other virus. Conclusion There was immune turbulence among patients with virus infection. Natural immunity, humoral immu- nity and cellular immunity all decreased while cellular immunity activated abnormally. The immune activation of the patients infected with Epstein - Barr virus was more obvious than that of patients infected with other virus.
