摘  要:目的:分析血管紧张素转换酶(ACE)基因在中山市0~3岁反复喘息患儿中单核苷酸多态性(SNP)分布特点并探讨其与儿童哮喘遗传易感性及反复喘息危险因素的关系。方法:采用聚合酶链反应产物电泳鉴定法及实时荧光定量核酸扩增技术比较分析ACE基因SNP在反复喘息患儿和普通上呼吸道感染对照者中的分布情况,判断ACE基因SNP分布与哮喘预测指数的相关性。采用单因素Logistic回归分析探讨与反复喘息明显相关的ACE基因DD基因型与反复喘息危险因素的相关性。结果:ACE基因D型等位基因频率在喘息组明显高于普通上呼吸道感染对照组,差异有统计学意义(P〈0.05);ACE基因位点纯合子基因型(DD)频率在哮喘预测指数(API)阳性组明显高于阴性组,差异有统计学意义(P〈0.05);在喘息组中占优势的ACE基因DD基因型与病毒感染史及支原体感染史有明显相关性。结论:中山地区携带ACE基因D型等位基因的婴幼儿更具有发生喘息的可能性,ACE基因DD基因型可能是本地区反复喘息的易感基因,DD基因型患儿更容易受病毒感染或支原体感染而发生反复喘息,而支原体感染较病毒感染引起喘息的风险性更大,基因与环境因素存在交互协同作用,促使喘息的发生。Objective: To analyze the distribution characteristics of single nucleotide polymorphism (SNP) of angiotensin converting enzyme (ACE) gene in 0 -3 -year- old children with recurrent wheezing in Zhongshan city, explore its relationship with genetic sus- ceptibility of childhood asthma and risk factors of recurrent wheezing. Methods : PCR product electrophoresis identification and real - time quantitative PCR nucleic acid amplification were used to compare and analyze the frequency distributions of SNPs of ACE gene in children with recurrent wheezing and common upper respiratory tract infection (control group), the correlation between SNP distribution of ACE gene and asthma predictive index was determined, univariate logistic regression analysis was used to explore the correlation between DD genotype of ACE gene and risk factors of recurrent wheezing. Results: The frequency of D allele of ACE gene in wheezing group was statistically sig- nificantly higher than that in control group (P 〈 0. 05 ), and the frequency of DD genotype of ACE gene in positive asthma predictive index (API) group was statistically significantly higher than that in negative API group (P 〈 0. 05 ) . DD genotype of ACE gene was dominant in wheezing group, which was significantly correlated with viral infection history and Mycoplasma infection history. Conclusion: The children with D allele of ACE gene are more prone to asthma in Zhongshan area. DD genotype of ACE gene may be the risk factor for recurrent wheezing in the area. The children with DD genotype of ACE gene are more prone to recurrent wheezing due to viral infection and Mycoplasma infection, especially Mycoplasrna infection, gene and environmental factors have interaction, they can promote the onset of wheezing.
