快速康复理念对老年胃肠恶性肿瘤患者术后医院感染影响的临床研究

摘　　要：目的研究快速康复理念对老年胃肠恶性肿瘤患者术后医院感染影响,以期寻找降低老年胃肠恶性肿瘤患者术后医院感染的方法,提高临床治疗水平。方法医院从2012年1月起在胃肠外科全面运用快速康复理念,至2013年1月共收治213例老年胃肠恶性肿瘤患者作为观察组,选取2011年1-12月收治的193例老年胃肠恶性肿瘤患者作为对照组,对相关影响因素进行分析,数据采用SPSS13.0分析。结果观察组213例患者发生感染共7例,感染率为3.29%,其中肺部感染4例、尿路感染2例、切口感染1例;对照组193例患者发生感染共47例感染率为24.35%,其中肺部感染25例、尿路感染17例、切口感染5例,均明显高于观察组,两组比较差异有统计学意义(P<0.05)。结论快速康复理念通过全方位的干预,使患者营养状况、体质、机体免疫力得到明显的提高,是降低医院感染发生率的重要措施之一,同时减轻患者的痛苦,促进其术后康复。OBJECTIVE To study effect of the concept of fast track surgery on postoperative nosocomial infections in elderly patients with gastrointestinal malignancies, in order to find ways to reduce postoperative nosocomial infections in elderly Patients with gastrointestinal malignancies and improve the level of clinical treatment. METHODS The concept of fast track surgery was comprehensively applied in the department of gastrointestinal surgery in our hospital since Jan. 2012. Totally 213 cases of elderly patients with gastrointestinal malignancies during Jan. 2012 to Jan. 2013 were taken as the observation group, and 193 elderly patients with gastrointestinal cancer patients admitted from Jan to Dee, 2011 as the control group, the relevant factors were analyzed, data were analyzed with SPSS 13.0. RESULTS There were 7 cases of infections （3.29 %） in the observation group, including 4 cases of pulmonary infections, 2 cases of urinary tract infections, 1 case of incision infection. There were 47 cases of infections （24.35%） in the control group, including 25 cases of lung infections, 17 cases of urinary tract infections, 5 cases of incision infections, significantly more than the observation group （P〈0. 05）. CONCLUSION With a full range of surgical intervention, the fast track surgery concept can markedly improve the patient nutritional status, physical fitness, and immunity, which is one of the important measures to reduce the incidence of nosocomial infections, the suffering of patients and promote their postoperative rehabilitation.
