α-硫辛酸联合血塞通治疗糖尿病周围神经病变的临床观察

摘　　要：目的:探讨α-硫辛酸联合血塞通在改善糖尿病周围神经病变(DPN)临床症状中的应用效果,为糖尿病周围神经病变提供中西医结合治疗的思路。方法:选择2011年8月—2012年8月接受治疗的98例糖尿病周围神经病变患者作为研究对象,46例患者静脉滴注甲钴胺治疗,将其视为对照组,52例患者静脉滴注血塞通以及α-硫辛酸,视为观察组,应用肌电图(EMG)记录两组患者治疗前后神经传导速度变化,根据神经病变主觉症状问卷(TSS)比较两组的临床症状改善效果。结果:两组治疗后下肢或足部疼痛、感觉异常、灼热感及麻木等主要症状评分均要好于治疗前,观察组的各项症状改善效果更加明显;肌电图检查结果显示观察组神经传导速度改善效果明显好于对照组,观察组总有效率为86.54%,对照组为76.09%,两组比较差异具有统计学意义(P<0.05)。结论:α-硫辛酸联合血塞通有效的改善了糖尿病周围神经病变的临床症状,促进了感觉和运动神经传导速度的恢复,治疗效果明显,值得临床推广应用。Objective ： To investigate the alpha lipoic acid combined with Xuesaitong in the improvement of diabetic peripheral neuropathy （DPN） the effect of application of clinical symptoms, treatment of traditional Chinese and Western medicine combined with the idea of providing for diabetic peripheral neuropathy. Methods ：98 cases of diabetic peripheral neuropathy patients from 2011 August -2012 year in August treated in our hospital as the object of study,46 patients with intravenous injection of mecobalamin, be regarded as the control group,52 patients with intravenous infusion of Xue-saitong and alpha lipoic acid, as the observation group, using electromyography （EMG） record of nerve conduction veloci- ty of two groups before and after treatment, according to the neuropathy symptoms questionnaire （TSS） and clinical symp- toms of the two groups were compared the improvement effect. Results： After treatment,leg or foot pain,paresthesia, bum- ing sensation and numbness and other major symptom scores of both groups were better than before treatment, and the symptoms improvement of observation group was more obvious. The EMG results showed that the improvement of nerve conduction velocity on observation group was better than the control group obviously. Total effective rate of observation group was 86.54% ,and 76.09% was the control group. There was a significant difference between groups by comparision （ P 〈 0.05 ）. Conclusion： alpha lipoic acid combined with Xuesaitong is effective in improving the clinical symptoms of diabetic peripheral neuropathy, promote the sensory and motor nerve conduction velocity recovery, the treatment effect is obvious, it is worthy of clinical application.
