低浓度肝素钠冲管液预防IABP管道阻塞的临床观察

摘　　要：了解低浓度肝素钠冲管液对急性心肌梗死合并心源性休克患者使用IABP球囊辅助治疗的管道阻塞及相关血栓事件、出血风险的情况。我们选取2009年1月～2013年9月急性心肌梗死合并心源性休克患者使用IABP球囊应用肝素钠冲管液患者70例,其中包括实验组35例：应用0.9%NS加如入肝素钠稀释至浓度为10 000 u/L,对照组35例：应用0.9%生理盐水加入肝素钠稀释至浓度为（25 000μ/L）。观察术后IABP管道通畅及下肢动脉血栓形成情况,出血事件发生率。两组均未发现管道堵塞及下肢动脉血栓形成情况,但实验组总体出血事件的发生率明显较少。提示：较低浓度肝素钠冲管液（10 000 u/L）与常规浓度冲管液（25 000μ/L）相比在急性心肌梗死合并心源性休克IABP术后相关血栓事件无明显增加,但后者的出血风险较低。Objective To study the events of pipe blocking and thrombosis and the risk of bleeding of the low concentration heparin sodium flushing fluid（SHF） in patients with acute myocardial infarction（AMI） and cardiogenic shock who were employed with intra- aortic ballon pump（ IABP） implantation. Methods 70 patients suffered from AMI and cardiogenic shock with IABP implantation during January 2009 and September 2003 were selected and divided into two groups. Experimental group（35 cases）was given 0. 9% normal saline solvent and heparin sodium with the concentration of 10 000 u /L,control group（35 cases） was given the concentration of 25 000 u /L. The postoperative effect of IABP pipe blocking and thrombosis of lower extremity were observed,and the rates of bleeding were estimated. Results Neither groups showed circumstances of pipe blocking and thrombosis of lower extremity,but the general incidence of bleeding of the experimental group was significantly lower. Conclusion The IABP implantation related thrombosis events with lower concentration of SHF（10 000 u/L） has no obvious change compared with the normal one（25 000 u/L） in patients with AMI and cardiogenic shock,but the latter has relatively lower risk of bleeding.
