扶正祛邪法对急性呼吸窘迫综合征患者促炎因子的影响

摘　　要：目的:观察扶正祛邪法对急性呼吸窘迫综合征(ARDS)血清促炎因子TNF-α、IL-1β、IL-6、IL-8的影响。方法:68例患者按住院的先后顺序,按随机分组法分为治疗组和对照组各34例。对照组采用呼吸机支持与西药治疗原发病;治疗组在对照组治疗基础上予扶正祛邪中药治疗。比较两组病死率、脱机率、撤机时间、住ICU时间;检测比较两组治疗前后血清促炎因子表达水平。结果:两组治疗后促炎因子较治疗前均下降(P<0.05),治疗组较对照组下降更明显(P<0.05);治疗组与对照组比较,能够明显缩短机械通气时间(P<0.05)。结论:扶正祛邪法中药可能通过抑制促炎因子表达治疗ARDS。Objective：To observe the effects of vital qi - strengthening and pathogen - eliminating traditional Chinese medicine （TCM） treatment on serum levels of tumor necrosis factor （TNF） -α, interleukin （IL） - 1β, IL -6, and IL- 8 in patients with acute respiratory distress syndrome （ARDS）. Methods：Sixty -eight patients were randomly divided into treatment group （n = 34 ） and control group （n = 34 ） in the order of admission. Both groups received respiratory support and Western medicine treatment;in addition, the treatment group received vital qi - strengthening and pathogen - eliminating TCM treatment. The two groups were compared in terms of mortality, ventilator weaning rate, ventilator weaning time, and duration of ICU stay. The serum levels of TNF - α, IL - β, IL -6, and IL -8 were determined before and after treatment. Results：Both groups showed significant decreases in proinflammatory factor levels after treatment （P 〈 0. 05 ）, and the treatment group had significantly higher decreases than the control group （P 〈 0.05 ）. Compared with the control group, the treatment group had a significantly shortened duration of mechanical ventilation （ P 〈 0. 05 ）. Conclusion- Vital qi - strengthening and pathogen - eliminating TCM treatment can inhibit the expression of proinflammatory factors in patients with ARDS.
