MED与开窗髓核摘除术治疗腰椎间盘突出症的疗效对比分析

摘　　要：目的探讨后路椎间盘镜髓核摘除术(MED,A组)与开窗髓核摘除术(B组)治疗腰椎间盘突出症(LDH)的临床疗效对比。方法回顾性分析我院2013年1月至2014年1月期间24例手术治疗LDH患者临床资料,按手术方式不同分为A组(12例)和B组(同期同组医师完成12例),比较2组术中出血量、手术时间、术后下床时间、住院时间及近期临床疗效。结果 A组和B组术中出血量、手术时间、术后卧床时间、住院时间比较,差异均有统计学意义(P<0.05);2组患者术前、术后1周、3个月、6个月的临床疗效比较,差异均无统计学意义(P>0.05)。结论 MED与开窗髓核摘除术治疗LDH近期临床疗效相似,但其在术中出血量、术后下床时间、住院时间方面更具优势,从而提高患者生活质量。Objective To compare the clinical efficacy between minimally invasive surgery and open discectomy intreating lumbar intervertebral disc herniation patients. Methods The materianl of 24 LDH patients was retrospectively analyzed in our hospital according to operation type that patients were divided into MED group and open discectomy group from 2013 January to 2014 January. A mount of bleeding operation,the operation time, the rest time on bed after operation,hospitalization time,and short duration clinical results were compared. Results The difference was statistically significant in bleeding operation,the operation time the rest time on bed after operation, hospitalization time in the MED group and open discectomy group（P〈0.05）.However,The difference of JOA score of preoperative,a week ,three months ,six months after surgery was not statistically significant （P〉0.05）. Conclusions MED group and open discectomy could both achieves satisfactory clinical efficacy in treating LDH patients,while the advantage of the MED was more obvious.
