摘 要:目的探讨老年上呼吸道感染患者感染病原菌分布及药敏试验结果,以期为临床诊治提供参考依据。方法对2013年1-8月医院587份上呼吸道感染患者痰液及咽拭子标本,采用法国生物梅里埃有限公司的VITEK-32全自动微生物分析系统进行菌株鉴定,以K-B纸片法进行药敏试验,所有数据采用SPSS17.0进行分析。结果 587例患者送检标本中共有368例病原菌培养阳性,阳性率为62.69%;培养出病原菌394株,其中革兰阴性菌319株、革兰阳性菌61株、真菌14株,分别占80.97%、15.48%、3.55%;前3位革兰阳性菌为凝固酶阴性葡萄球菌、金黄色葡萄球菌、肺炎链球菌,分别占6.35%、4.57%、2.54%;凝固酶阴性葡萄球菌、金黄色葡萄球菌、肺炎链球菌对临床多种抗菌药物耐药性较高,对氨苄西林/舒巴坦、万古霉素、多西环素、夫西地酸耐药性较低。结论老年上呼吸道感染患者感染病原菌以革兰阴性为主,病原菌对临床常用抗菌药物耐药现象普遍。OBJECTIVE To investigate the distribution and drug susceptibility test results of pathogenic bacteria in elderly patients with upper respiratory tract infections,in order to provide reference for clinical diagnosis and treatment.METHODS A total of 587 sputum and throat swab test specimens of patients with upper respiratory tract infections were collected during Jan-Aug.2013 and analyzed using the VITEK-32 Automatic Microbe Analysis System(BioMérieux SA,France)to identify the pathogen strains,medicine sensitive tests were conducted using K-B testpaper method,and all the data were analyzed using SPSS17.0.RESULTS Out of the 587 cases specimens,368 cases of specimens were pathogenic bacteria culture positive,the positive rate was 62.69%.A total of 394 strains of pathogens were cultivated,of which 319 strains were gram-negative bacteria,61 strains were grampositive bacteria,14 were fungi,accounting for 80.97%,15.48%,3.55%,respectively.The top 3gram-positive bacteria were coagulase-negative Staphylococci,Staphylococcus aureus,Streptococcus pneumoniae,accounting for 6.35%,4.57%,2.54%,respectively.Coagulase-negative Staphylococci,S.aureus,S.pneumoniae showed relatively high resistance to multiple clinical antibiotics while the resistance to ampicillin/subactam,vancomycin,doxycycline,and fusidic acid was relatively low.CONCLUSION In elderly patients with upper respiratory tract infections,the pathogenic bacteria were dominated by gram-negative bacteria,and the pathogens showed resistance against the common clinical antibiotics.
